一例重症脑炎所致严重精神障碍患者使用子弹头鼻肠管空肠营养的临床效果观察

摘　　要：目的本文主要对重症脑炎所致严重精神障碍患者在进行肠内营养支持治疗中,观察使用子弹头双腔鼻肠管的临床效果.方法通过对一例重症脑炎所致严重精神障碍患者在使用镇静药物期间合并呼吸抑制,无法经口进食,使用盲插管置入子弹头鼻肠管进行空肠喂养后的营养指标、胃液反流、肺部感染转归等情况进行分析、总结,从而观察其在严重精神疾病患者中的临床实用性.结果与结论帮助患者顺利度过呼吸抑制高风险期,该患者因精神障碍致吞咽困难、拒食而营养不良,使用子弹头鼻肠管置管进行空肠营养,加强营养支持治疗26天,大幅度提升其机体营养情况,缩短总病程,改善胃肠反流,降低反流误吸、呼吸机相关肺炎的发生几率,对重症脑炎合并严重精神障碍患者的治疗及康复具有积极作用,值得临床借鉴与推广.Objective This article mainly observed the clinical effect of using simplified bullet double cavity nasoenteric tube in enteral nutrition support therapy for patients with severe mental disorders caused by severe encephalitis.Methods A patient with severe mental disorder caused by severe encephalitis was observed during the use of sedative drugs combined with respiratory depression,unable to eat through mouth,and the nutritional indexes,gastric reflux,pulmonary infection and other outcomes of jejunal feeding with blind intubation into a simple bullet naso-intestinal tube were analyzed and summarized,so as to observe its clinical practicability in patients with severe mental diseases.Results and Conclusion To help patients successfully through the high-risk period of respiratory depression,dysphagia and food refusal caused by mental disorders and malnutrition,jejunal nutrition with bullet naso-intestinal tube was used,and nutritional support treatment was strengthened for 26 days to greatly improve the body nutrition,shorten the total course of disease,improve gastrointestinal reflux,and reduce the incidence of reflux aspiration and ventilators related pneumonia.It has a positive effect on the treatment and rehabilitation of patients with severe encephalitis complicated with serious mental disorders,and is worthy of clinical reference and promotion.
