"临床用药"标准对麻醉药品标准化管理以及药学服务质量的影响

摘　　要：目的探讨"临床用药"标准应用于麻醉药品管理中对麻醉药品不合理用药以及药学服务质量的影响.方法调取"临床用药"标准实施前(2022年4月-2023年3月)和实施后(2023年4月-2024年6月)南通市如东县人民医院门诊麻醉处方以及麻醉药品住院医嘱,标准实施前门诊处方点评1573例,住院医嘱点评1386例;实施后门诊处方点评1720例,住院医嘱点评1560例.分析临床用药标准实施前后麻醉药品处方问题;比较标准实施前后麻醉药品住院医嘱点评合格率,麻醉药品门诊处方点评合格率,以及肿瘤科麻醉性镇痛药的用药频率.结果标准实施后,麻醉药品门诊处方问题及住院医嘱问题均有所降低;标准实施后,麻醉药品住院医嘱点评合格率、麻醉药品门诊处方点评合格率均高于实施前(P<0.05);标准实施前,肿瘤科麻醉性镇痛药用药频度(DDDs)前3位为芬太尼透皮贴、硫酸吗啡缓释片、阿桔片,标准实施后,芬太尼透皮贴、盐酸羟考酮缓释片DDDs均降低,阿桔片DDDs相对上升.结论"临床用药"标准应用于麻醉药品管理中有助于麻醉药品规范化管理和麻醉性镇痛药规范使用,提升麻醉药品药学服务质量.Objective To investigate the impact of the"Clinical Medicin"standards implemented in narcotic drug management on irrational medication practices and the quality of pharmaceutical care.Methods Access to outpatient anesthesia prescriptions and inpatient orders for narcotic drugs before the implementation of the"Clinical Medicin"standard(April 2022 to March 2023)and after the implementation(April 2023 to June 2024),before the implementation of the standard,1573 outpatient prescriptions and 1386 inpatient orders were reviewed,after implementation,1720 outpatient prescriptions and 1560 inpatient orders were reviewed.The prescription problems of narcotic drugs before and after the implementation of clinical drug standards were analyzed.Compare the reasonableness rate of comments on inpatient medical orders and outpatient prescriptions of narcotic drugs before and after the implementation of the standard,and the frequency of narcotic analgesics in oncology.Results Following the implementation of the standard,there was a notable reduction in issues related to outpatient narcotic drug prescriptions and inpatient medical orders.Following the implementation of the standard,the rate of reasonableness in comments on inpatient orders for narcotic drugs and outpatient prescriptions for narcotic drugs significantly improved compared to the period prior to its implementation(P<0.05).Compared with before the implementation of the standard,the use frequency of narcotic analgesics top 3 use frequency were fentanyl transdermal patch,morphine sulfate sustained release tablet,azithromycin tablets,after the implementation of the standard,the DDDs of fentanyl transdermal patches and oxycodone hydrochloride sustained-release tablets decreased,while the DDDs of azithromycin tablets increased relatively.Conclusion The application of"clinical drug use"standard in the management of narcotic drugs is helpful to the standardized management of narcotic drugs and the standardized use of narcotic analgesics,and improve the quality of pharmaceutica
