慢性萎缩性胃炎生化指标与舌象参数及典型症状关联研究

摘　　要：目的:探讨慢性萎缩性胃炎患者的血液生化指标与舌象参数以及中医典型症状之间的关联,为中医诊疗客观化提供科学依据。方法:用课题组前期研制的中医(脾胃系)问诊信息采集表记录患者的中医典型症状等信息,舌图像及参数提取由手机、色卡、中医舌面象采集分析系统完成。通过Spearman相关性方法,分析各参数之间的关联。结果:白蛋白、总胆红素、直接(结合)胆红素、间接胆红素、谷草转氨酶、谷草转氨酶线粒体同工酶、谷丙转氨酶、γ-谷氨酰转肽酶、甘胆酸、淀粉样蛋白A、前白蛋白、低密度脂蛋白胆固醇与部分舌象参数之间呈正相关(P<0.05);直接(结合)胆红素、谷草转氨酶/谷丙转氨酶、碱性磷酸酶、胆汁酸、前白蛋白、胱甘肽还原酶、高密度脂蛋白胆固醇、尿素氮与部分舌象参数之间呈负相关(P<0.05)。谷草转氨酶/谷丙转氨酶、高密度脂蛋白胆固醇与脘腹痛呈正相关(P<0.05);球蛋白与嗳气/呃逆、泛酸呈负相关(P<0.05),淀粉样蛋白A与腹胀、脘腹痞满呈负相关(P<0.05)。结论:慢性萎缩性胃炎患者舌象参数及中医典型症状可以反映血液生化指标改变。随着疾病进展,血液生化指标变化,患者舌色偏暗红或青紫,苔色偏黄腻,脘腹痛及嗳气/呃逆、泛酸症状加重。Objective:To explore the association between blood biochemical indicators and tongue parameters as well as typical symptoms of Chinese medicine in patients with chronic atrophic gastritis,so as to provide scientific basis for the objectivization of Chinese medicine diagnosis and treatment.Methods:The information collection form of Chinese medicine(spleen and stomach system) consultation developed by the group in the early stage was used to record the typical symptoms of Chinese medicine and other information of the patients,and the extraction of tongue images and parameters was completed by the phone,color card,and Chinese medicine tongue image collection and analysis system.The association between parameters was analyzed by Spearman correlation method.Results:Albumin,total bilirubin,conjugated bilirubin,indirect bilirubin,glutamic oxaloacetic transaminase,mitochondrial aspartate aminotransferase,glutamic pyruvic transaminase,γ-glutamyl transpeptidase,cholyglycine,pre-albumin,Amyloid A and low-density lipoprotein cholesterol were positively correlated with tongue parameters(P<0.05).Conjugated bilirubin,glutamic oxaloacetic transaminase/glutamic pyruvic transaminase,alkaline phosphatase,bile acids,pre-albumin,glutathione reductase,high-density lipoprotein cholesterol and urea nitrogen were negatively correlated with tongue parameters(P<0.05).Glutamic oxaloacetic transaminase/glutamic pyruvic transaminase and high-density lipoprotein cholesterol were positively correlated with epigastric pain(P<0.05).Globulin was negatively correlated with belching/eructation and pantothenic acid(P<0.05),and amyloid A was negatively correlated with bloating and epigastric fullness(P<0.05).Conclusion:Tongue parameters and typical symptoms of Chinese medicine in patients with chronic atrophic gastritis can reflect changes in blood biochemical indicators.As the disease progresses,the blood biochemical indicators change,the patient's tongue color is dark red or greenish purple,the moss color is yellowish and greasy,and the symptoms of
