摘  要:针刺作为传统的中医疗法具有较好的镇痛效果,穴位局部是针刺效应启动的关键环节。文章通过梳理总结,发现针刺可通过激活Aδ纤维和C纤维上的瞬时受体电位香草酸(TRPV)1通道直接引起穴位局部神经末梢兴奋,将针刺信息向中枢传递;针刺也可通过激活TRPV1通道活化MAPK信号通路促进成纤维细胞骨架重构和细胞增殖,或通过激活TRPV2通道促进肥大细胞脱颗粒,活化的成纤维细胞或肥大细胞可释放ATP和腺苷等关键分子,启动CWP级联反应,经末梢向中枢传递针刺信息。而针刺引起的穴位局部神经末梢兴奋和炎性反应可能进一步敏化TRPV1通道活性,维持针刺信息传递。阐明了TRPV蛋白在针刺效应启动中的作用机制,以期为针灸的临床应用提供理论依据。As a traditional Chinese medicine therapy,acupuncture has a good analgesic effect,and the local acupoint is the key link in the initiation of acupuncture effect.In this paper,it is found that acupuncture can directly excite the local nerve endings of acupoints by activating TRPV1 channels on Aδ fibers and C fibers,and transmit acupuncture information to the center.Acupuncture can also promote fibroblast cytoskeleton remodeling and cell proliferation by activating the TRPV1 channel to activate the MAPK signaling pathway,or promote mast cell degranulation by activating the TRPV2 channel.Activated fibroblasts or mast cells can release key molecules such as ATP and adenosine,initiate the CWP cascade reaction,and transmit acupuncture information to the central nervous system through the peripheral.The excitement and inflammatory response of local nerve endings caused by acupuncture may further sensitize the activity of TRPV1 channel and maintain the transmission of acupuncture information.The mechanism of TRPV protein in the initiation of acupuncture effect was clarified,which provided a theoretical basis for the clinical application of acupuncture.
