摘 要:目的探析在慢性阻塞性肺疾病急性加重(acute exacerbation of chronic obstructive pulmonary disease,AECOPD)患者无创通气间歇期应用经鼻高流量氧疗的效果。方法随机选取2022年2月—2024年2月高州市人民医院收治的70例AECOPD患者作为研究对象,依据不同治疗方法分为两组,各35例。对照组于无创正压通气间歇期实施鼻导管氧疗,观察组于无创正压通气间歇期实施经鼻高流量氧疗。对比两组动脉血气指标、呼吸功能、生活质量、临床相关指标和并发症发生率。结果观察组无创通气间歇歇息次数少于对照组,且呼吸支持时间、每日无创通气治疗时间及重症监护室入住时间均短于对照组,差异均有统计学意义(P均<0.05)。观察组动脉二氧化碳分压低于对照组,动脉血氧分压、氧合指数高于对照组,差异均有统计学意义(P均<0.05)。观察组圣乔治呼吸问卷评分为(42.73±4.06)分、生活质量评分为(64.48±6.16)分,均低于对照组的(48.66±4.42)分、(75.24±6.57)分,差异均有统计学意义(t=5.845,7.068;P均<0.05)。两组并发症总发生率对比,差异无统计学意义(P>0.05)。结论AECOPD患者无创通气间歇期实施经鼻高流量氧疗治疗,可缩短呼吸支持和治疗时间,改善动脉血气指标、呼吸功能,提高生活质量,且临床有效性和安全性确切。Objective To explore the effect of nasal high flow oxygen therapy in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)during the intermittent period of non-invasive ventilation intermittent period.Methods A total of 70 AECOPD patients admitted to Gaozhou People's Hospital from February 2022 to February 2024 were randomly selected as the research objects.According to different treatment methods,they were divided into two groups,with 35 cases in each group.The control group was treated with nasal catheter oxygen therapy during the intermittent period of non-invasive positive pressure ventilation,and the observation group was treated with nasal high flow oxygen therapy during the intermittent period of non-invasive positive pressure ventilation.The arterial blood gas indexes,respiratory function,quality of life,clinical related indexes and incidence of complications were compared between the two groups.Results The number of intermittent breaks of non-invasive ventilation in the observation group was less than that in the control group,and the respiratory support time,daily non-invasive ventilation treatment time and intensive care unit hospitalization time were shorter than those in the control group,the differences were statistically significant(all P<0.05).The partial pressure of carbon dioxide in the observation group was lower than that in the control group,and the arterial partial oxygen pressure,oxygenation index were higher than those in the control group,the differences were statistically significant(all P<0.05).The observation group had lower Saint George's Breathing Questionnaire score as(42.73±4.06)points and quality of life score as(64.48±6.16)points than(48.66±4.42)points and(75.24±6.57)points in the control group,and the differences were statistically significant(t=5.845,7.068;both P<0.05).There was no significant difference in the total incidence of complications between the two groups(P>0.05).Conclusion The implementation of nasal high flow oxygen therapy in patien
