老年心房颤动合并不稳定型心绞痛治疗中达比加群酯联合替格瑞洛的应用效果分析

摘　　要：目的旨在探究达比加群酯联合替格瑞洛对老年房颤动合并不稳定心绞痛患者的治疗效果。方法回顾性选取2023年1月—2024年3月晋江市医院(上海市第六人民医院福建医院)收治的104例心房颤动合并不稳定型心绞痛老年患者的临床资料,根据不同治疗方式分为对照组和观察组,分别使用华法林钠片联合替格瑞洛片治疗、达比加群酯联合替格瑞洛片治疗,各52例。分析两组患者治疗前后凝血功能和心肌损伤标志物水平的变化,以及临床疗效和不良反应的发生率。结果治疗后,观察组在凝血功能和心肌损伤标志物水平均优于对照组,差异均有统计学意义(P均<0.05)。相比于观察组的98.08%(51/52),对照组治疗总有效率[84.62%(44/52)]更低,差异有统计学意义(χ^(2)=4.379,P<0.05)。观察组不良反应总发生率低于对照组,差异有统计学意义(P<0.05)。结论对老年心房颤动合并不稳定型心绞痛的患者采用达比加群酯和替格瑞洛的联合治疗方案,可以取得更好的治疗效果,改善凝血功能,减少不良反应。Objective The aim is to explore the therapeutic effect of dabigatran etexilate combined with ticagrelor on elderly patients with atrial fibrillation and unstable angina pectoris.Methods The clinical data of 104 elderly patients with atrial fibrillation and unstable angina pectoris admitted to Jinjiang Hospital(Shanghai Sixth People's Hospital Fujian Hospital)from January 2023 to March 2024 were retrospectively selected and grouped according to different treatment methods.Participants were divided into control group and observation group based on the treatment regimens employed.The control group was treated with warfarin sodium plus ticagrelor,whereas the observation group was given dabigatran etexide in combination with ticagrelor.There were 52 cases in each group.The analysis focused on the differences in coagulation and cardiac injury markers before and after treatment in both groups.It also considered the clinical benefits and the frequency of adverse reactions.Results After treatment,compared with the control group,the coagulation function and the level of myocardial injury markers of the observation group was better,the differences were statistically significant(both P<0.05).The observation group's total treatment effectiveness was notably higher at 98.08%(51/52)compared to the control group's rate of 84.62%(44/52),and the difference was statistically significant(χ^(2)=4.379,P<0.05).Adverse reactions were less frequent in the observation group than in the control group,and the difference was statistically significant(P<0.05).Conclusion Dabigatran etexilate combined with ticagrelor in the treatment of elderly patients with atrial fibrillation complicated with unstable angina pectoris can achieve better results,which can improve coagulation function and reduce adverse reactions.
