摘  要:目的探讨封闭负压引流术(vacuum sealing drainage,VSD)联合微动力负压护创敷料对深Ⅱ度烧伤创面微循环及创面愈合的影响。方法选择2021年11月-2022年11月收治的60例深Ⅱ度烧伤患者,以随机数字表法随机分为干预组、对照组,各30例。对照组开展微动力负压护创敷料换药,观察组在对照组的基础上加用VSD治疗。应用独立样本t检验比较两组平均换药次数、住院时间、创面愈合时间。应用卡方检验比较两组创面红细胞聚集情况、血管清晰程度、血流灌注量与并发症发生情况。应用独立样本t检验和重复测量方差分析比较两组的创面愈合率。结果干预组平均换药次数、住院时间、创面愈合时间均少于对照组(P<0.05)。治疗后7、14、21天,干预组创面愈合率均高于对照组(P<0.05),而治疗后28天,两组创面愈合率比较无差异(P>0.05)。干预组红细胞无聚集率、血管清晰率、血流灌注量均高于对照组(P<0.05)。干预组并发症总发生率低于对照组(P<0.05)。结论VSD联合微动力负压护创敷料治疗深Ⅱ度烧伤,可减少换药次数、住院时间,改善创面微循环,促进创面愈合,且不会增加并发症发生率,值得临床应用。Objective To investigate effect of vacuum sealing drainage(VSD)combined with microdynamic negative pressure wound dressing on wound microcirculation and healing of deep burns.Methods Sixty patients with deep-degree burns admitted to our hospital from November 2021 to November 2022 were selected.They were randomized by random numbers table and organized into intervention and control groups,with 30 patients each.The control group changed the microdynamic negative pressure invasive dressing,and the intervention group had the additional VSD procedure.Independent sample t-test was applied to compare the mean dressing changes,hospital stay,and wound healing time between the two groups.Chi-square test was used to compare the red cell aggregation,vascular clarity,blood perfusion and the occurrence of complications between the two groups.Independent samples t-test and repeated measures ANOVA were applied to compare wound healing rates between the two groups.Results The mean number of dressing changes,hospital stay,and wound healing time in the intervention group were less than that in the control group(P<0.05).At 7,14 and 21 d after treatment,the wound healing rate of the intervention group was higher than that of the control group(P<0.05),while there was no dfference between the two groups at 28 d after treatment(P>0.05).Unpacked rate,vascular clarity rate,and blood perfusion rate were higher than those of the control group(P<0.05).The overall incidence of complications in the intervention group was lower than that in the control group(P<0.05).Conclusion VSD combined with microdynamic negative pressure wound dressing to treat deep burn reduced the number of dressing changes and hospital stay,improved wound microcirculation,promoted wound healing,and without increased complications.Therefore,the combined procedure can be useful for clinical application.
