摘 要:目的:观察Club细胞分泌蛋白-16(CC16)与慢性阻塞性肺疾病(COPD)中性粒细胞气道炎症的关系,探讨其对COPD临床症状和肺功能的影响。方法:研究对象为我院2023年1月至2023年10月入院的COPD患者。通过留取急性加重期和稳定期患者血清和痰液标本,ELISA方法检测血清和痰上清液CC16,以及痰液中性粒细胞弹性蛋白酶(NE)、白介素-8(IL-8)和黏蛋白MUC5AC;评估COPD患者急性加重期症状,包括咳嗽程度评分(CET)、COPD患者自我评估测试(CAT)和改良版英国医学研究委员会(mMRC)呼吸困难问卷;检测稳定期患者肺功能。结果:与稳定期相比,急性加重期COPD患者气道CC16显著减低,而MUC5AC、NE和IL8浓度明显增高;并且气道CC16与MUC5AC、NE和IL8呈显著负相关,而MUC5AC与NE和IL8呈显著正相关。COPD患者急性加重期入院时症状与气道CC16浓度、呈明显负相关,而与气道MUC5AC浓度呈明显正相关。COPD患者急性期加重期血清CC16与症状无明显相关性,但稳定期血清CC16与肺功能呈明显正相关。结论:COPD急性加重期气道CC16减少与中性粒细胞气道炎症及其所致的气道黏液高分泌密切相关,也对其临床症状加重有重要影响。血清CC16还与稳定期肺功能密切相关。Objective:To investigate the relationship between club cell secretion protein 16(CC16)and airway inflammation mediated by neutrophils in chronic obstructive pulmonary disease(COPD),and to explore the effect of CC16 on clinical symptoms and lung function of COPD.Methods:The subjects of COPD patients were admitted to our hospital from January to October in 2023.Serum and sputum samples of patients in COPD exacerbation and stable stage were collected respectively,and CC16 in serum and sputum supernatant,neutrophil elastic(NE),interleukin-8(IL-8)and MUC5AC in sputum supernatant were detected by ELISA.Assessing symptoms of patients in COPD exacerbations,including cough Severity Score(CET),COPD Assessment Test(CAT)and the modified Medical Research Council(mMRC)dyspnea scale.Lung function was measured in stable stage.Results:Compared with stable COPD patients,CC16 decreased significantly in exacerbations,while MUC5AC,NE and IL8 concentrations increased significantly in airways.Airways CC16 was negatively correlated with MUC5AC,NE and IL8,while MUC5AC was positively correlated with NE and IL8.The symptoms of COPD patients were negatively correlated with airway CC16 concentration and positively correlated with airway MUC5AC concentration.There was no significant correlation between serum CC16 and symptoms in COPD patients with exacerbations,but there was a significant positive correlation between serum CC16 and pulmonary function in stable COPD patients.Conclusion:CC16 of sputum is closely related to airway inflammation and airway mucus hypersecretion mediated by neutrophil in COPD exacerbation,and also has an important impact on clinical symptoms.Serum CC16 is also significantly related to lung function in stable COPD.
