摘　　要：目的:对比经皮微创椎弓根钉内固定术与开放椎弓根钉内固定术治疗胸腰椎骨折的效果。方法:选取常州市第四人民医院骨科2019年1月—2022年12月收治的80例胸腰椎骨折患者为研究对象,随机分为观察组与对照组,各40例。观察组行经皮微创椎弓根钉内固定术治疗,对照组行开放椎弓根钉内固定术治疗。对比两组治疗效果、手术指标、关节功能、伤椎恢复情况、并发症发生情况。结果:观察组治疗总有效率高于对照组(P=0.025);观察组切口长度短于对照组,出血量少于对照组,视觉模拟评分法评分低于对照组,住院时间短于对照组(P<0.001);治疗后,两组Oswestry功能障碍指数问卷表评分降低,Harris评分升高,观察组优于对照组(P<0.05);治疗后,两组伤椎前缘高度比值、伤椎后缘高度比值升高,伤椎后凸Cobb角减小,观察组优于对照组(P<0.05);观察组并发症总发生率低于对照组(P=0.025)。结论:经皮微创椎弓根钉内固定术治疗胸腰椎骨折的效果优于开放椎弓根钉内固定术,可提高手术质量,增强关节功能,促进伤椎恢复,且安全性更高。Objective:To compare the effect of percutaneous minimally invasive pedicle nail fixation and open pedicle nail fixation in the treatment of thoracolumbar fracture.Methods:Eighty patients with thoracolumbar fracture admitted to the Department of Orthopedics of Changzhou Fourth People's Hospital from January 2019 to December 2022 were selected as the study objects,and were randomly divided into observation group and control group,with forty cases in each group.The observation group was treated with percutaneous minimally invasive pedicle nail fixation,and the control group was treated with open pedicle nail fixation.The therapeutic effect,surgical indexes,joint function,recovery of injured vertebrae and complications were compared between the two groups.Results:The total effective rate of observation group was higher than that of control group(P=0.025).The length of the incision in the observation group was shorter than that in the control group,the amount of bleeding was less than that in the control group,the visual analog score was lower than that in the control group,and the hospitalization time was shorter than that in the control group(P<0.001).After treatment,the score of Oswestry disability index questionnaire decreased and the Harris score increased in both groups,and the observation group was better than the control group(P<0.05).After treatment,the ratio of the height of the injured vertebral front and the height of the injured posterior edge of the two groups increased,and the Cobb angle of the injured kyphosis decreased,and the observation group was better than the control group(P<0.05).The total complication rate of observation group was lower than that of control group(P=0.025).Conclusion:The effect of minimally invasive percutaneous pedicle nail internal fixation in the treatment of thoracolumbar fracture is better than that of open pedicle nail internal fixation,which can improve the surgical quality,enhance the joint function,promote the recovery of the injured vertebra,and have higher safety.
