血清干扰素-γ、诱导蛋白-10和单核细胞趋化蛋白-1联合检测对肺结核的诊断价值

摘  要:目的:评估血清中干扰素-γ(INF-γ)、诱导蛋白-10(IP-10)和单核细胞趋化蛋白-1(MCP-1)水平在肺结核诊断中的应用价值。方法:选取2023年3月—2024年3月东莞市第九人民医院收治的150例肺结核患者纳入试验组,选取同时期该医院收治的150例肺部非结核分枝杆菌感染患者纳入对照1组,选取同时期150例健康体检者纳入对照2组。检测研究对象血清INF-γ、IP-10和MCP-1水平,采用受试者工作特征曲线(ROC)分析各指标及其联合检测诊断肺结核的效能。结果:试验组IFN-γ、IP-10、MCP-1水平高于对照1组与对照2组,对照1组IFN-γ、IP-10水平高于对照2组(P<0.05)。ROC分析结果显示,IFN-γROC曲线下面积(AUC)最大(P<0.001),IFN-γ、IP-10和MCP-1联合检测的AUC大于各指标单独检测,其中IFN-γ、IP-10和MCP-1三项指标联合检测的AUC最大(P<0.001)。结论:血清INF-γ、IP-10和MCP-1可作为诊断肺结核的标志物,联合检测具有更高的诊断效能。Objective:To evaluate the diagnostic value of serum levels of interferon-γ(INF-γ),induced-protein-10(IP-10)and monocyte chemotactic protein-1(MCP-1)in pulmonary tuberculosis.Methods:A total of one hundred and fifty patients with pulmonary tuberculosis admitted to Dongguan Ninth People's Hospital from March 2023 to March 2024 were included in the experimental group,one hundred and fifty patients with pulmonary non-tuberculosis mycobacteria admitted to the hospital during the same period were included in control group 1,and one hundred and fifty healthy physical examination patients during the same period were included in control group 2.Serum INF-γ,IP-10 and MCP-1 levels of the subjects were detected,and receiver operating characteristic curve(ROC)was used to analyze each index and the efficiency of combined detection in diagnosis of pulmonary tuberculosis.Results:The levels of IFN-γ,IP-10 and MCP-1 in experimental group were higher than those in control group 1 and control group 2,and the levels of IFN-γand IP-10 in control group 1 were higher than those in control group 2(P<0.05).ROC analysis results showed that the area under ROC curve(AUC)of IFN-γwas the largest(P<0.001),and the AUC of IFN-γ,IP-10 and MCP-1 combined detection was greater than that of each indicator alone,among which the AUC of IFN-γ,IP-10 and MCP-1 combined detection was the largest(P<0.001).Conclusion:Serum INF-γ,IP-10 and MCP-1 can be used as markers for the diagnosis of pulmonary tuberculosis,and the combined detection has higher diagnostic efficacy.
