基于“病痰饮者,当以温药和之”思想浅析精液不液化症的证治思路

摘　　要：精液不液化症是男科常见病症之一,归属于中医“精凝”“精瘀”“不育”等范畴,临床表现为精液胶着黏稠的异常状态。“病痰饮者,当以温药和之”出自《金匮要略》痰饮篇,文章分析其“温药和之”的核心思想,并归纳不液化之精液与痰饮在外观质地、病因病机、致病特点上存在的相似之处,异病同治,提出精液不液化症的治疗应借鉴“和”的思想,以求“和”为原则,重在梳理气机,调和阴阳,使机体气化功能回归正常,在此基础上辨证论治,恢复精液正常生理状态。Non-liquefaction of semen,falling into the categories of seminal coagulation,seminal stasis and infertility in traditional Chinese medicine(TCM),is one of the common diseases in andrology,whose clinical manifestation is the abnormal state of sticky semen.Recorded in the Chapter of Phlegm-fluid Retention from Zhang Zhongjing's Synopsis of the Golden Chamber(Jin Gui Yao Lue),patients with phlegm-fluid retention should be harmonized with warm medicinals.In this article,we analyzed the core idea of"To harmonize with warm medicinals",generalized the similarities between non-liquefied semen and phlegm in appearance,texture,etiology,pathogenesis and pathogenic characteristics.Based on the thought of homotherapy for heteropathy,we believe that the treatment of seminal non-liquefaction should be based on seeking harmony,that is,focusing on smoothing the qi movement and regulating yin and yang to restore the qi transformation of the human body.Through treatment based on syndrome differentiation,the normal physiological state of semen will be finally achieved.
