摘　　要：目的探究急性痛风性关节炎(GA)患者血清双链脱氧核糖核酸(dsDNA)水平及其与代谢和炎症指标的相关性分析。方法本研究为病例对照研究,选取2021年12月至2022年8月北部战区总医院风湿免疫科收治的30例高尿酸血症(AHU)男性患者为AHU组,年龄33.5(30.0,37.3)岁,年龄范围为18~70岁;选取同时期风湿免疫科收治的30例GA男性患者为GA组,年龄33.0(26.0,39.0)岁,年龄范围为18~70岁。同时选取同时期于北部战区总医院行健康检查的30名男性健康志愿者为对照组,年龄34.5(28.8,46.5)岁,年龄范围为18~70岁。采集三组的基本信息、生化指标和全血标本,比较三组的空腹血糖、总胆固醇、甘油三酯、血尿酸、红细胞沉降率(ESR)、C-反应蛋白(CRP)、丙氨酸转氨酶(ALT)、血清肌酐、肌酐清除率(Ccr)、dsDNA的浓度、白细胞介素-18(IL-18)和白细胞介素-1β(IL-1β)水平,分析GA患者血清dsDNA的影响因素。结果三组的年龄、空腹血糖、ALT比较,差异无统计学意义(P>0.05)。AHU组的体质量指数[BMI,(27.4±3.2)kg/m2]、总胆固醇[(5.2±1.0)mmol/L]、甘油三酯[2.0(1.1,3.3)mmol/L]、血尿酸[(513.5±60.5)μmol/L]、白细胞计数[(6.68±1.63)×10^(9)/L]、ESR[4.0(2.0,6.3)mm/h]和Ccr[(131.0±29.4)ml/min]显著高于对照组[(23.9±2.3)kg/m2、(4.0±1.0)mmol/L、0.8(0.7,1.4)mmol/L、(355.1±38.4)μmol/L、(5.80±1.21)×10^(9)/L、3.00(2.00,11.25)mm/h、(109.3±18.3)ml/min],GA组的BMI[(28.2±2.9)kg/m2]、总胆固醇[(5.3±0.9)mmol/L]、甘油三酯[1.8(1.4,2.8)mmol/L]、血尿酸[(535.4±97.0)μmol/L]、白细胞计数[(8.85±1.74)×10^(9)/L]、ESR[16.0(8.0,29.0)mm/h]和Ccr[(126.0±31.3)ml/min]高于对照组,GA组的血小板计数[(276.0±50.7)×10^(9)/L]、CRP[(23.3±2.9)mg/L]和血清肌酐[(92.5±17.0)μmol/L]高于对照组[(226.2±46.6)×10^(9)/L、(3.0±0.2)mg/L、(82.8±9.2)μmol/L],GA组的白细胞计数[(8.85±1.74)×10^(9)/L]、血小板计数[(276.0±50.7)×10^(9)/L]、ESR[16.0(8.0,29.0)mm/h]、CRP[(23.3±2.9)mg/Objective:To explore the serum double-strand deoxyribonucleic acid(dsDNA)levels in patients with acute gouty arthritis and its influencing factors.Methods:This study was a case-control study,a total of 30 male patients with asymptomatic hyperuricemia(AHU)were selected from the department of Rheumatology and Immunology of General Hospital of Northern Theater Command from December 2021 to August 2022 as the AHU group,aged 33.5(30.0,37.3)years old,the age ranging from 18 to 70 years old;a total of 30 male patients from the department of Rheumatology and Immunology with gouty arthritis(GA)were selected as the GA group,aged 33.0(26.0,39.0)years old,ranging from 18 to 70 years old.Additionally,30 healthy male volunteers who underwent health checks at the General Hospital of Northern Theater Command during the same period were selected as the control group,aged 34.5(28.8,46.5)years old,the age ranging from 18 to 70 years old.Basic information,biochemical indicators,and whole blood samples were collected from the three groups to compare fasting blood glucose(FBG),total cholesterol,triglycerides,serum uric acid,erythrocyte sedimentation rate(ESR),C-reactive protein(CRP),alanine aminotransferase(ALT),serum creatinine,creatinine clearance rate(Ccr),dsDNA concentration,interleukin-18(IL-18),and interleukin-1β(IL-1β)levels,and to analyze the influencing factors of serum dsDNA in GA patients.Results:There were no significant differences in age,FBG,and ALT among the three groups(P>0.05).The body mass index[BMI,(27.4±3.2)kg/m 2],total cholesterol[(5.2±1.0)mmol/L],triglycerides[2.0(1.1,3.3)mmol/L],serum uric acid[(513.5±60.5)μmol/L],white blood cell count[(6.68±1.63)×10^(9)/L],ESR[4.0(2.0,6.3)mm/h],and Ccr[(131.0±29.4)ml/min]in AHU group were significantly higher than the control group[(23.9±2.3)kg/m 2,(4.0±1.0)mmol/L,0.8(0.7,1.4)mmol/L,(355.1±38.4)μmol/L,(5.80±1.21)×10^(9)/L,3.00(2.00,11.25)mm/h,(109.3±18.3)ml/min],the BMI[(28.2±2.9)kg/m 2],total cholesterol[(5.3±0.9)mmol/L],triglycerides[1.8(1.4,2.8)mmol/L],se
