血液透析联合沙库巴曲缬沙坦治疗心衰伴肾功能不全的疗效

摘　　要：目的探讨血液透析滤过联合沙库巴曲缬沙坦片治疗心力衰竭合并肾功能不全患者的N末端B型脑钠肽前体(NT-proBNP)水平、射血分数(EF)以及血清肌酐(Scr)的影响。方法将2022年2月至2023年2月期间江西省赣州市第五人民医院就诊的60例患者按照治疗方案不同分为对照组和观察组,其中男34例(56.67%),女26例(43.33%),对照组(n=30)给予血液透析滤过治疗,观察组(n=30)再增加沙库巴曲缬沙坦治疗,观察两组NT-proBNP水平、Scr水平以及心功能。结果治疗后,两组患者NT-proBNP水平均较对照组水平低,且观察组水平较对照组水平低(P<0.05);两组血清肌酐(Scr)水平均较治疗前降低,且观察组水平较对照组水平低(P<0.05);两组左室射血分数(LVEF)水平均较治疗前升高,且观察组水平高于对照组(P<0.05);两组左心室舒张末期内径(LVEDD)、左心室收缩末期内径(LVESD)水平均较治疗前降低,且观察组水平较对照组水平低(P<0.05)。结论血液透析滤过联合沙库巴曲缬沙坦应用于心力衰竭合并肾功能不全患者的治疗能够有效控制其NT-proBNP水平,有利于心肾功能的改善。Objective To investigate the effects of hemodialysis filtration combined with Sacubitril Valsartan tablets on N-terminal B-type brain natriuretic peptide precursor(NT proBNP)levels,ejection fraction(EF),and serum creatinine(Scr)inpatients with heart failure and renalinsufficiency.Methods Sixtypatients whoreceived treatmentat the Fifth People's Hospital of Ganzhou City,Jiangxi Province from February 2022 to February 2023 were divided into a control group and an observation group according to different treatment plans.Among them,34 were male(56.67%)and 26 were female(43.33%).The control group(n=30)received hemodialysis filtration treatment,while the observation group(n=30)received additional treatment with Sacubitril and Valsartan.The NT proBNP levels,Scr levels,and heart function were observed in both groups.Results After treatment,the NT proBNP levels in both groups of patients were lower than those in the control group,and the observation group had lower levels than the control group(P<0.05).The serum creatinine(Scr)levels in both groups decreased compared to before treatment,and the observation group had lower levels than the control group(P<0.05);The left ventricular ejection fraction(LVEF)levels in both groups increased compared to before treatment,and the observation group had higher levels than the control group(P<0.05);The levels of left ventricular end diastolic diameter(LVEDD)and left ventricular end systolic diameter(LVESD)in both groups decreased compared to before treatment,and the levels in the observation group were lower than those in the control group(P<0.05).Conclusions The combination ofhemodialysisfiltration and Sacubitril Valsartan can effectively control the NT proBNP level in patients with heartfailure and renal in sufficiency,which is beneficial for the improvement of cardiac and renal function.
