摘　　要：目的分析急性心肌梗死(AMI)恶性室性心律失常(MVA)中监测24小时动态心电图参数的临床应用价值。方法临床纳入2022年9月至2023年4月在本院收治的急性心肌梗死(AMI)病人120例为研究组,同期选择体检检查者110例为对照组,全部研究病例均24小时动态心电图监测,研究组按照有无出现恶性心律失常分组,分为恶性组(n=56)、非恶性组(n=64),分析研究组、对照组的QT间期变异性(QTV)、心率变异性(HRV);分析恶性组、非恶性组的QT间期变异性(QTV)、心率变异性(HRV)。结果与对照组相比,研究组的Night-QTV、Day-QTV、24h-QTV、r MSSD、SDANN-index、SDNN-index、SDNN指标均显著降低,差异有统计意义(P<0.05);与非恶性组相比,恶性组的Night-QTV、Day-QTV、24h-QTV、r MSSD、SDANN-index、SDANN-index、SDNN指标均显著降低,差异有统计意义(P<0.05)。结论急性心肌梗死监测24小时动态心电图对于评估恶性室性心律失常具有较高的临床评估价值,值得临床推荐应用。Objective To analyze the clinical application value of monitoring 24h Holter parameters in malignant ventricular arrhythmia(MVA)of acute myocardial infarction(AMI).Methods One hundred and twenty patients with acute myocardial infarction(AMI)admitted in our hospital from September 2022 to April 2023 were selected as the study group,and 110 physical examiners were selected as the control group at the same time.All the study cases were monitored by 24h Holter.The study group was divided into malignant group(n=56)and non malignant group(n=64)according to whether malignant arrhythmia occurred.The QTV of the study group and the control group were analyzed Heart rate variability(HRV).The QT interval variability(QTV)and heart rate variability(HRV)of malignant group and non malignant group were analyzed.Results Compared with the control group,the indexes of night QTV,day QTV,24h QTV,RMSSD,SDANN index,SDNN index and SDNN in the study group were significantly decreased,and the difference was statistically significant(P<0.05).Compared with the non malignant group,the indexes of night QTV,day QTV,24h QTV,RMSSD,SDANN index,SDANN index and SDNN in the malignant group were significantly lower(P<0.05).Conclusions Twenty-four-hour Holter monitoring of acute myocardial infarction has high clinical evaluation value for the evaluation of malignant ventricular arrhythmia,which is worthy of clinical recommendation.
