摘  要:目的观察室间隔增厚的心电图变化并探讨其临床意义。方法选择本院92例经超声证实室间隔增厚的超声心动图,观察其心电图变化并测量其V_(1)~V_(3)导联R波电压,并与相应的室间隔厚度及正常均值做对比。结果心电图逆钟向转位伴或不伴T波改变50例,顺钟向转位伴或不伴T波改变34例,无钟向转位伴或不伴T波改变8例。样本中V_(2)、V_(3)导联R波波幅平均值均高于正常平均值,这个现象在重度逆钟向转位当中更加明显;V_(3)导联R波电压随室间隔增厚略有相应的增加。结论室间隔增厚的心电图表现有多种,逆钟向转位占比最大,并且逆钟向转位中的重度逆钟向转位的V_(2)、V_(3)导联R波波幅明显高于正常均值。代表前间隔部除极导联的电压,V_(3)导联和室间隔的厚度呈弱正相关,V_(1)、V_(2)导联无相关性。Objective To observe the electrocardiogram changes of ventricular septal thickening and explore its clinical significance.Methods Ninety-two cases of echocardiography confirmed ventricular septal thickening in our hospital were selected,and their electrocardiogram changes were observed.The R-wave voltage in lead V_(1)~V_(3)was measured,and compared with the corresponding ventricular septal thickness and normal mean.Results The electrocardiogram showed that there were 50 cases of counterclockwise transposition with or without T wave changes,34 cases of clockwise transposition with or without T wave changes,and 8 cases of no clockwise transposition with or without T wave changes.The average R-wave amplitude of leads V_(2)and V_(3)in the sample is higher than the normal average,which is more pronounced in severe counterclockwise transposition.The V_(3)lead R-wave voltage slightly increases correspondingly with the thickening of the interventricular septum.Conclusions There are various electrocardiographic manifestations of ventricular septal thickening,with the highest proportion of retrograde transposition.In severe retrograde transposition,the V_(2)and V_(3)lead R-wave amplitudes are significantly higher than the normal mean.The voltage representing the depolarization lead in the anterior septum is weakly positively correlated with the thickness of the interventricular septum in lead V_(3),while there is no correlation in leads V_(1)and V_(2).
