腺样体低温等离子消融术与内镜下刮除术治疗小儿鼾症的效果比较

摘　　要：目的 比较腺样体低温等离子消融术与内镜下刮除术治疗小儿鼾症的效果。方法 选取2022年6月至2023年10月广西壮族自治区妇幼保健院耳鼻咽喉头颈外科收治的350例鼾症患儿作为研究对象,采用随机数字表法分为对照组(175例)和试验组(175例)。对照组采用内镜下刮除术治疗,试验组采用腺样体低温等离子消融术治疗,比较两组临床疗效、手术情况、睡眠呼吸暂停低通气指数(AHI)、最低血氧饱和度(LSaO2)、匹兹堡睡眠质量指数(PSQI)、儿童阻塞性睡眠呼吸暂停低通气综合征特异性生活质量调查量表(OSA-18)评分、并发症总发生率、术后6个月随访的复发率。结果 试验组手术时间、疼痛持续时间均短于对照组(P<0.05)。试验组术中出血量少于对照组(P<0.05)。试验组治疗总有效率高于对照组(P<0.05)。术前,两组AHI、LSaO2以及PSQI、OSA-18评分比较,差异无统计学意义(P>0.05);术后1周,两组AHI、PSQI以及OSA-18评分均低于术前,LSaO2高于术前,且试验组AHI、PSQI以及OSA-18评分低于对照组,LSaO2高于对照组(P<0.05)。试验组并发症总发生率、术后随访6个月复发率均低于对照组(P<0.05)。结论 相较于内镜下刮除术,腺样体低温等离子消融术治疗小儿鼾症的效果更好,能够减少术中出血量,促进术后康复,提高睡眠和生活质量,减少术后并发症和复发。Objective To compare the effect of adenoid hypothermia plasma ablation and endoscopic curettage in the treatment of pediatric snoring.Methods A total of 350 children with snoring in Department of Otolaryngology Head and Neck Surgery,Maternity and Child Health Care of Guangxi Zhuang Autonomous Region from June 2022 to October 2023 were selected.They were divided into control group(175 cases)and experimental group(175 cases)by random number table method.The control group was treated with endoscopic curettage,while the experimental group was treated with adenoid hypothermia plasma ablation.The clinical efficacy,operation status,sleep apnea hypopnea index(AHI),lowest blood oxygen saturation(LSaO2),Pittsburgh sleep quality index(PSQI)and children’s obstructive sleep apnea syndrome-18(OSA-18)score,total complication rate and recurrence rate at 6 months after postoperative followup were compared between the two groups.Results The operation time and pain duration in the experimental group were shorter than those in the control group(P<0.05).The intraoperative bleeding volume in the experimental group was less than that in the control group(P<0.05).The total effective rate of experimental group was higher than that of control group (P < 0.05). Before operation, there were no significant differences between the two groups in AHI, LSaO2 and the scores of PSQI and OSA-18 (P < 0.05). One week after operation, the AHI and the scores of PSQI and OSA- 18 in both groups were lower than those before operation, LSaO2 was higher than that before operation, and AHI, PSQI and OSA-18 scores in experimental group were lower than those in the control group, LSaO2 was higher than that in the control group (P < 0.05). The total complication rate and recurrence rate at 6 months after postoperative follow-up in the experimental group were lower than those in the control group (P < 0.05).
