摘 要:目的 分析左甲状腺素钠(LT4)对妊娠糖尿病合并亚临床甲状腺功能减退症患者子代短期神经心理发育的影响。方法 选取2021年8月至2022年12月兰州市妇幼保健院收治的33例妊娠糖尿病合并亚临床甲状腺功能减退患者为观察组,选取同期33例妊娠期健康女性为对照组,其中观察组均采用LT4治疗。比较两组甲状腺功能[促甲状腺激素(TSH)、游离甲状腺素(FT4)、甲状腺过氧化物酶抗体(TPO-Ab)],以及新生儿出生42d和出生3、6个月的身高、体重和子代短期神经心理发育情况。结果 观察组TSH、TPOAb水平高于对照组(P<0.05);两组FT4水平比较,差异无统计学意义(P>0.05)。两组新生儿出生42 d及出生3、6个月的身高、体重比较,差异无统计学意义(P> 0.05)。两组子代短期神经心理发育比较,差异无统计学意义(P>0.05)。结论 妊娠糖尿病合并亚临床甲状腺功能减退患者接受LT4治疗后,子代短期神经心理发育与正常妊娠期女性的子代短期神经心理发育无差异。Objective To analyze the effect of Levothyroxine Sodium(LT4)on short-term neuropsychological development in offspring of patients with gestational diabetes mellitus complicated with subclinical hypothyroidism.Methods A total of 33 patients with gestational diabetes mellitus complicated with subclinical hypothyroidism admitted to Lanzhou Maternal and Child Health Hospital from August 2021 to December 2022 were selected as the observation group,and 33 healthy pregnant women during the same period were selected as the control group.The observation group was treated with LT4.Thyroid function(thyroid stimulating hormone[TSH],free thyroxine[FT4],thyroid peroxidase antibody[TPO-Ab]),neonatal height and weight at 42 days,3 and 6 months after birth,as well as short-term neuropsychological development of offspring were compared between the two groups.Results The levels of TSH and TPO-Ab in the observation group were higher than those in the control group(P<0.05).There was no significant difference in FT4 level between the two groups(P>0.05).There were no statistically significant differences in the height and weight of newborns at 42 days, 3 and 6 months after birth between the two groups (P > 0.05). There was no significant difference in short-term neuropsychological development between the two groups (P > 0.05). Conclusion There is no significant difference in the short-term neuropsychological development between the offspring of women with gestational diabetes mellitus complicated with subclinical hypothyroidism treated with LT4 and the offspring of women with normal pregnancy.
