超声引导下聚桂醇注射辅助清宫术在剖宫产子宫瘢痕妊娠中的应用效果

摘　　要：目的 探讨超声引导下聚桂醇注射辅助清宫术治疗剖宫产子宫瘢痕妊娠(CSP)的临床效果。方法选取习水县人民医院2022年1月至2023年10月收治的60例CSP患者,以随机数字表法分为两组,各30例。对照组行子宫动脉栓塞术联合清宫术治疗,观察组行超声引导下聚桂醇注射辅助清宫术治疗。比较两组临床相关指标、术后恢复情况、卵巢储备功能及并发症。结果 观察组手术时间、住院时间短于对照组(P <0.05);两组术中出血量比较无差异(P> 0.05);观察组术后恢复情况优于对照组(P <0.05);观察组卵泡刺激素水平低于对照组,雌二醇、抗米勒管激素水平高于对照组,并发症总发生率低于对照组(P<0.05)。结论 超声引导下聚桂醇注射辅助清宫术治疗CSP效果更佳,可促进术后恢复,对卵巢功能影响小,且并发症少。Objective To investigate the clinical effect of ultrasound-guided Lauromacrogol injection assisted uterine curettage in the treatment of cesarean scar pregnancy(CSP).Methods A total of 60 patients with CSP admitted to the Xishui County People’s Hospital from January 2022 to October 2023 were selected and divided into two groups by random number table method,with 30 cases in each group.The control group was treated with uterine artery embolization combined with uterine curettage,and the observation group was treated with ultrasound-guided Lauromacrogol injection assisted uterine curettage.The clinical indicators,postoperative recovery,ovarian reserve function and complications were compared between the two groups.Results The operation time and hospitalization time in the observation group were shorter than those in the control group(P<0.05).There was no difference in intraoperative blood loss between the two groups(P>0.05).The postoperative recovery of the observation group was better than that of the control group(P<0.05).The level of follicle stimulating hormone in the observation group was lower than that in the control group,the levels of estradiol and anti-Müllerian hormone were higher than those in the control group,and the total incidence of complications was lower than that in the control group(P<0.05).Conclusion Ultrasound-guided Lauromacrogol injection assisted uterine curettage is more effective in the treatment of CSP,which can promote postoperative recovery,has little effect on ovaria n function and fewer complications.
