摘　　要：目的探讨孕早期血清铁蛋白浓度对妊娠糖尿病(GDM)的预测价值。方法选取2021年6月至2023年12月在枣庄市妇幼保健院分娩的42例GDM孕妇作为病例组,另选取同期50例健康孕妇作为对照组进行回顾性分析。两组均于孕早期(孕11~13周)抽取空腹静脉血,采用葡萄糖氧化酶法测定空腹血糖,采用电化学法测定血清铁蛋白浓度。采用受试者工作特征(ROC)曲线评估孕早期血清铁蛋白浓度对GDM的预测价值。结果病例组孕早期血清铁蛋白浓度高于对照组(P<0.05)。病例组孕早期血清铁蛋白浓度对GDM预测价值的ROC曲线下面积为0.909(P=0.001),约登指数为0.666,其对应的血清铁蛋白浓度界值为41.8ng/ml,灵敏度、特异度分别为97.6%、69.0%。结论孕早期血清铁蛋白浓度对GDM的发病存在一定的预测价值,可为GDM的预防和监测提供临床依据。Objective To explore the predictive value of serum ferritin concentration in early pregnancy on gestational diabetes mellitus(GDM).Methods A total of 42 pregnant women with GDM who gave birth in Maternity and Child Health Care of Zaozhuang from June 2021 to December 2023 were selected as the case group,and 50 healthy pregnant women in the same period were selected as the control group for retrospective analysis.Both groups took fasting venous blood in early pregnancy(11 to 13 weeks),and fasting blood glucose was measured by glucose oxidase method,and serum ferritin concentration was measured by electrochemical method.The predictive value of serum ferritin concentration in early pregnancy on GDM was evaluated by the receiver operating characteristic(ROC)curve.Results The serum ferritin concentration in early pregnancy of the case group was higher than that of the control group(P<0.05).The area under the curve of the predictive value of serum ferritin concentration in early pregnancy on GDM was 0.909(P=0.001).The Youden index was 0.666,the corresponding serum ferritin concentration threshold was 41.8 ng/ml,and the sensitivity and specificity were 97.6%and 69.0%,respectively.Conclusion The serum ferritin concentration in early pregnancy has certain predictive value for the onset of GDM,and can provide clinical basis for the prevention and monitoring of GDM.
