秦艽地黄通痹汤治疗湿热蕴毒型类风湿关节炎活动期的临床效果

摘　　要：目的:探讨湿热蕴毒型类风湿关节炎(RA)活动期患者给予秦艽地黄通痹汤治疗的临床效果。方法:选取2023年3—11月尤溪县总医院收治的60例湿热蕴毒型RA活动期患者作为研究对象,以随机数表法分为研究组(n=30,秦艽地黄通痹汤联合甲氨蝶呤治疗)与对照组(n=30,甲氨蝶呤治疗)。比较两组临床疗效、中医症候积分、实验室指标、疼痛程度及关节疾病活动度。结果:研究组治疗总有效率为93.33%,较对照组的70.00%更高,差异有统计学意义(P<0.05);治疗后,两组肿胀、口渴、晨僵、热痛积分降低,且研究组较对照组更低,差异有统计学意义(P<0.05);治疗后,两组类风湿因子(RF)、C反应蛋白(CRP)、红细胞沉降率(ESR)降低,且研究组较对照组更低,差异有统计学意义(P<0.05);治疗后,两组视觉模拟评分法(VAS)评分、28个关节疾病活动(DAS28)评分降低,且研究组较对照组更低,差异有统计学意义(P<0.05)。结论:秦艽地黄通痹汤治疗湿热蕴毒型RA活动期患者,有助于提高临床效果,改善临床症状、疼痛程度及关节疾病活动度,降低RF、CRP、ESR水平。Objective:To explore the clinical effect of Qinjiao Dihuang Tongbi Decoction in the treatment of active rheumatoid arthritis(RA)patients with damp heat accumulation toxin type.Method:A total of 60 patients with active RA with damp heat accumulation toxin type who admitted in Youxi County General Hospital from March to November 2023 were selected as the study objects,and they were randomly divided into the study group(n=30,Qinjiao Dihuang Tongbi Decoction combined with Methotrexate treatment)and the control group(30 cases,Methotrexate treatment)by random number table method.The clinical efficacy,traditional Chinese medicine syndrome score,laboratory indicators,pain level and joint disease activity in two groups were compared.Result:The total effective rate of treatment in the study group was 93.33%,which was higher than 70.00%in the control group,and the difference was statistically significant(P<0.05);after treatment,the scores of swelling,thirst,morning stiffness and heat pain in two groups were decreased,and those in the study group were lower than the control group,the differences were statistically significant(P<0.05);after treatment,the rheumatoid factor(RF),C-reactive protein(CRP)and erythrocyte sedimentation rate(ESR)in two groups were decreased,and those in the study group were lower than the control group,the differences were statistically significant(P<0.05);after treatment,the visual analogue scale(VAS)scores and 28 joint disease activity(DAS28)scores in two groups were decreased,and those in the study group were lower than the control group,the differences were statistically significant(P<0.05).Conclusion:Qinjiao Dihuang Tongbi Decoction is helpful to improve the clinical effect,improve the clinical symptoms,pain degree and joint disease activity,and reduce the levels of RF,CRP and ESR in patients with active RA with damp heat accumulation toxin type.
