桃红四物汤加减联合温针灸治疗膝关节滑膜炎的临床观察

摘　　要：目的:观察桃红四物汤加减联合温针灸治疗膝关节滑膜炎的临床效果。方法:选取2022年3月—2023年3月青海红十字医院收治的80例膝关节滑膜炎患者作为研究对象,按照随机数表法分为研究组(n=35)与对照组(n=45)。两组均给予常规治疗,对照组采用温针灸治疗,研究组在对照组基础上联用桃红四物汤加减治疗,比较两组临床疗效、中医症候积分、炎症因子[C反应蛋白(CRP)、红细胞沉降率(ESR)、肿瘤坏死因子-α(TNF-α)]、临床症状改善情况[视觉模拟评分法(VAS评分)、Lysholm量表(Lysholm评分)、膝关节活动度]及不良反应发生情况。结果:治疗结束后,研究组治疗总有效率高于对照组,差异有统计学意义(P<0.05);治疗结束后,两组中医症候积分、VAS评分及CRP、ESR和TNF-α水平均降低,且研究组低于对照组,两组Lysholm评分、膝关节活动度升高,且研究组高于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:在常规治疗基础上,桃红四物汤加减联合温针灸治疗膝关节滑膜炎患者能提高临床效果,改善临床症状,提高膝关节功能和活动度,减轻关节炎症刺激,治疗安全性较高。Objective:To observe the clinical effect of Modified Taohong Siwu Decoction combined with warm acupuncture in the treatment of knee synovitis.Method:A total of 80 patients with knee synovitis who admitted to Qinghai Red Cross Hospital from March 2022 to March 2023 were selected as the research objects,they were divided into the study group(n=35)and the control group(n=45)according to the random number table method.Two groups were given routine treatment,the control group was treated with warm acupuncture,the study group was treated with Modified Taohong Siwu Decoction on the basis of the control group,the clinical efficacy,traditional Chinese medicine symptom score,inflammatory factors[C-reactive protein(CRP),erythrocyte sedimentation rate(ESR),tumor necrosis factor-α(TNF-α)],improvement of clinical symptoms[visual analogue scale(VAS score),Lysholm scale(Lysholm score),knee joint activity]and adverse reactions were compared between two groups.Result:After treatment,the total effective rate of the study group was higher than that of the control group,and the difference was statistically significant(P<0.05);after treatment,the traditional Chinese medicine symptom scores,VAS scores and the levels of CRP,ESR and TNF-αin two groups were decreased,and those in the study group were lower than those in the control group,the Lysholm scores and knee joint activity in two groups were increased,and those in the study group were higher than those in the control group,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between two groups(P>0.05).Conclusion:On the basis of conventional treatment,Modified Taohong Siwu Decoction combined with warm acupuncture in the treatment of knee synovitis can improve the clinical effect,improve the clinical symptoms,improve knee joint function and activity,reduce the joint inflammation stimulation,and has high treatment safety.
