左炔诺孕酮宫内节育系统辅助治疗子宫内膜息肉手术患者的效果

摘　　要：目的:分析左炔诺孕酮宫内节育系统辅助治疗子宫内膜息肉手术患者的效果。方法:选取2019年1月—2021年12月在北京丰台医院妇产科经宫腔镜手术确诊的80例子宫内膜息肉手术患者,以术后不同辅助治疗方法分组,对照组40例采用地屈孕酮片辅助治疗,观察组40例采用左炔诺孕酮宫内节育系统辅助治疗,比较两组的临床疗效、子宫内膜厚度、内分泌指标、月经症状、经期、复发情况。结果:治疗后,观察组的总有效率为95.00%,对照组为80.00%,观察组总有效率更高,差异有统计学意义(P<0.05)。治疗后3个月、6个月、12个月,两组子宫内膜的厚度显著小于治疗前,且观察组均小于对照组,差异有统计学意义(P<0.05)。治疗后3个月、6个月、12个月,两组黄体生成素(LH)、雌二醇(E2)及卵泡刺激素(FSH)水平与治疗前对比,差异无统计学意义(P>0.05);且两组不同时间各指标对比,差异无统计学意义(P>0.05)。治疗后3个月、6个月、12个月,两组月经症状、经期均较治疗前改善,且观察组的月经失血图(PBAC)分数更低,经期更短,差异有统计学意义(P<0.05)。治疗后12个月,观察组复发率为0,对照组复发率为15.00%,观察组复发率更低,差异有统计学意义(P<0.05)。结论:左炔诺孕酮宫内节育系统辅助治疗子宫内膜息肉手术患者有助于提高患者临床效果,降低子宫内膜厚度,减轻内分泌指标的影响,改善月经症状与经期,同时减少复发。Objective:To analyze the effect of Levonorgestrel-releasing Intrauterine System for adjuvant treatment of patients undergoing surgery for endometrial polyps.Method:A total of 80 cases of patients with endometrial polyps undergoing surgery diagnosed by hysteroscopic in the Obstetrics and Gynecology Department of Beijing Fengtai Hospital from January 2019 to December 2021 were selected.40 cases of patients in the control group were received adjuvant treated with Dydrogesterone Tablets,and 40 cases of patients in the observation group were received adjuvant treatment with Levonorgestrel-releasing Intrauterine System.The clinical efficacy,endometrial thickness,endocrine indexes,menstrual symptoms,menstrual period and recurrence were compared between the two groups.Result:After treatment,the total effective rate was 95.00%in the observation group,and the total effective rate of the control group was 80.00%,the total effective rate of the observation group was higher,the difference was statistically significant(P<0.05).At 3 months,6 months and 12 months after treatment,the endometrial thickness of the two groups were smaller than those before treatment,and the endometrial thickness in the observation group were smaller than those in the control group,the differences were statistically significant(P<0.05).There were no significant differences in luteinizing hormone(LH),estradiol(E2)and follicle stimulating hormone(FSH)levels between the two groups at 3 months,6 months and 12 months after treatment(P>0.05);and there were no significant differences in the indexes between the two groups at different time(P>0.05).At 3 months,6 months and 12 months after treatment,both menstrual symptoms and menstrual period in the two groups were improved compared with those before treatment,and the score of pictorial blood loss assessment chart(PBAC)was lower and menstrual period was shorter in the observation group,the differences were statistically significant(P<0.05).At 12 months after treatment,the recurrence rate was 0 in the observat
