摘 要:目的:探讨初发Graves病患者25羟维生素D[25(OH)D]表达及其与甲状腺抗体的关系。方法:选取2023年1—9月厦门市海沧医院收治的114例初发Graves病患者为研究对象,均进行血清25(OH)D检测,根据检查结果分为25(OH)D正常组、25(OH)D不足组、25(OH)D缺乏组。均给予针对性治疗,记录不同25(OH)D水平患者甲状腺抗体[促甲状腺激素受体抗体(TRAb)、甲状腺过氧化物酶抗体(TPOAb)、甲状腺球蛋白抗体(TGAb)]差异,应用Pearson相关性分析25(OH)D与甲状腺抗体的相关性。结果:根据检测结果,25(OH)D正常组38例,25(OH)D不足组42例,25(OH)D缺乏组34例。三组TGAb相比,差异无统计学意义(P>0.05);三组TRAb、TPOAb相比,差异有统计学意义(P<0.05);25(OH)D缺乏组TRAb、TPOAb较25(OH)D不足组高,25(OH)D不足组TRAb、TPOAb较25(OH)D正常组高,差异有统计学意义(P<0.05)。三组TRAb、TPOAb、25(OH)D差值比较,差异有统计学意义(P<0.05)。Pearson相关性分析显示,25(OH)D水平与甲状腺抗体水平呈负相关(r=-0.468、-0.395,P<0.05)。结论:初发Graves病患者25(OH)D水平与甲状腺抗体水平呈负相关,而初发Graves病患者25(OH)D充足利于治疗后甲状腺抗体水平改善。Objective:To investigate the expression of 25 hydroxyvitamin D[25(OH)D]in patients with newly diagnosed Graves disease and its relationship with thyroid antibodies.Method:From January to September 2023,114 newly diagnosed Graves disease patients admitted to Xiamen Haicang Hospital were selected as the study subjects,all of whom underwent serum 25(OH)D testing.According to the test results,they were divided into 25(OH)D normal group,25(OH)D not enough group,and 25(OH)D deficiency group.Targeted treatment was given to all patients,the differences of thyroid antibodies[thyrotropin receptor antibody(TRAb),thyroid peroxidase antibody(TPOAb),thyroglobulin Antibody(TGAb)]in patients with different 25(OH)D levels were recorded,and the correlation between 25(OH)D and thyroid antibodies was analyzed by Pearson correlation analysis.Result:According to the test results,there were 38 cases in 25(OH)D normal group,42 cases in 25(OH)D not enough group,and 34 cases in 25(OH)D deficiency group.There was no significant difference in TGAb among the three groups(P>0.05).TRAb and TPOAb were significantly different among the three groups(P<0.05).The TRAb and TPOAb of 25(OH)D deficiency group were higher than those of 25(OH)D not enough group,and the TRAb and TPOAb of 25(OH)D not enough group were higher than those of 25(OH)D normal group,and the differences were statistically significant(P<0.05).The difference of TRAb,TPOAb and 25(OH)D among the three groups was statistically significant(P<0.05).According to Pearson correlation analysis,the 25(OH)D level was negative correlation with TRAb and TPOAb(r=-0.468,-0.395,P<0.05).Conclusion:There is a negative correlation between 25(OH)D level and thyroid antibody levels in patients with newly diagnosed Graves disease,and sufficient 25(OH)D level in patients with early-onset Graves disease are beneficial for improving thyroid antibody levels after treatment.
