乙酰半胱氨酸雾化联合罗红霉素分散片对重症支原体肺炎患儿的影响  

Effect of Acetylcysteine Atomization Combined with Roxithromycin Dispersible Tablets on Children with Severe Mycoplasma Pneumonia

摘  要:目的:探讨乙酰半胱氨酸(AC)雾化联合罗红霉素分散片对重症支原体肺炎(MP)患儿的影响。方法:回顾性分析2022年5月—2023年9月于莆田市第一医院治疗的82例重症MP患儿的临床资料,根据不同治疗方式分为对照组(罗红霉素分散片治疗,41例)与观察组(AC雾化联合罗红霉素分散片治疗,41例)。比较两组临床症状改善时间、血气指标、免疫指标、炎症因子及不良反应。结果:治疗后,观察组咳嗽、咽痛、乏力、发热改善时间早于对照组,差异有统计学意义(P<0.05);治疗后,两组动脉血氧分压(PaO_(2))、血氧饱和度(SaO_(2))较治疗前升高,且观察组高于对照组,两组动脉血二氧化碳分压(PaCO_(2))、免疫球蛋白A(IgA)、免疫球蛋白G(IgG)、免疫球蛋白M(IgM)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、降钙素原(PCT)均较治疗前降低,且观察组低于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:重症MP患儿中采用AC雾化联合罗红霉素分散片治疗可有效缩短症状改善时间,改善血气指标及炎症因子,增强机体免疫力,且未增加不良反应。Objective:To explore the effect of Acetylcysteine(AC)atomization combined with Roxithromycin Dispersible Tablets on children with severe Mycoplasma pneumonia(MP).Method:The clinical data of 82 children with severe MP who treated in the First Hospital of Putian City from May 2022 to September 2023 were retrospectively analyzed,they were divided into control group(Roxithromycin Dispersible Tablets,41 cases)and observation group(AC atomization combined with Roxithromycin Dispersible Tablets,41 cases)according to different treatment methods.The improvement times of clinical symptoms,blood gas indicators indexes,immune indexes,inflammatory factors and adverse reactions were compared between two groups.Result:After treatment,the improvement times of cough,sore throat,fatigue and fever in the observation group were earlier than that in the control group,the differences were statistically significant(P<0.05);after treatment,the arterial partial pressure of oxygen(PaO_(2))and saturation of blood oxygen(SaO_(2))in the two groups were increased compared with those before treatment,and those in the observation group were higher than those in the control group,the levels of arterial partial pressure of carbon dioxide(PaCO_(2)),immunoglobulin A(IgA),immunoglobulin G(IgG),immunoglobulin M(IgM),interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)and procalcitonin(PCT)in the two groups were decreased compared with those before treatment,and those in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05);there was no significant difference in the incidence of adverse reactions between two groups(P>0.05).Conclusion:AC atomization combined with Roxithromycin Dispersible Tablets in children with severe MP can significantly shorten the improvement times of clinical symptoms,quickly regulate blood gas indexes and inflammatory factor levels,improve body's immunity,and do not increase adverse reactions.
