摘 要:目的:探讨鼻塞式持续正压通气治疗新生儿肺炎患儿的效果。方法:选取2022年3月—2024年1月普宁市妇幼保健计划生育服务中心收治的126例新生儿肺炎患儿作为研究对象,根据随机数表法分为研究组(n=69)和对照组(n=57)。对照组应用常规头罩吸氧法,研究组采取鼻塞式持续正压通气,比较两组临床疗效、临床症状改善时间、住院时间、血气指标及炎症因子。结果:研究组临床总有效率高于对照组,退热时间、气促消失时间、肺部阴影消失时间及住院时间均优于对照组,差异有统计学意义(P<0.05);治疗后,两组氧分压(PaO_(2))、氧合指数高于治疗前,二氧化碳分压(PCO_(2))、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、C反应蛋白(CRP)低于治疗前,且研究组PaO_(2)、氧合指数较对照组更高,PaCO_(2)、TNF-α、IL-6及CRP较对照组更低,差异有统计学意义(P<0.05)。结论:鼻塞式持续正压通气治疗新生儿肺炎患儿效果显著,可缓解临床症状,改善血气指标及炎症因子水平。Objective:To analyze the effect of nasal continuous positive pressure ventilation in the treatment of neonatal pneumonia in children.Method:A total of 126 children with neonatal pneumonia who admitted to Puning Maternal and Child Health and Family Planning Service Center from March 2022 to January 2024 were selected as the research objects,they were divided into the study group(n=69)and the control group(n=57)according to the random number table method.The control group was treated with conventional hood oxygen inhalation,and the study group was treated with nasal continuous positive pressure ventilation.The clinical efficacy,clinical symptom improvement time,hospitalization time,blood gas indexes and inflammatory factors were compared between two groups.Result:The total clinical effective rate in the study group was higher than that of the control group,and the antipyretic time,shortness of breath disappearance time,lung shadow disappearance time and hospitalization time were better than those of the control group,the differences were statistically significant(P<0.05);after treatment,the oxygen partial pressure(PaO_(2))and oxygenation index of two groups were higher than those before treatment,partial pressure of carbon dioxide(PaCO_(2)),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),C-reactive protein(CRP)were lower than those before treatment,and PaO_(2) and oxygenation index of the study group were higher than those of the control group,PaCO_(2),TNF-α,IL-6 and CRP were lower than those of the control group,the differences were statistically significant(P<0.05).Conclusion:Nasal continuous positive pressure ventilation is effective in the treatment of neonatal pneumonia in children,which can relieve clinical symptoms,and improve blood gas indexes and inflammatory factors.
