萆薢泽兰饮联合洛索洛芬钠治疗急性痛风性关节炎湿热蕴结证的临床研究

摘　　要：目的:评价萆薢泽兰饮联合洛索洛芬钠治疗急性痛风性关节炎湿热蕴结证的临床疗效。方法:纳入急性痛风性关节炎湿热蕴结证患者112例,随机分为治疗组与对照组各56例。对照组在饮食和生活方式调整的基础上口服洛索洛芬钠片,治疗组在对照组治疗基础上加用萆薢泽兰饮口服,疗程为7 d。治疗前后记录视觉模拟量表(VAS)评分、炎症因子水平[C反应蛋白(CRP)、血沉(ESR)]、血尿酸(UA)、受累关节肌肉骨骼超声检查OMERACT痛风病变半定量超声评分(超声评分)及中医证候积分,观察总有效率及不良反应情况。结果:治疗后两组患者VAS评分、炎症因子水平和中医证候积分均明显改善,且治疗组改善优于对照组(P<0.05);UA、超声评分比较,治疗后对照组降低不明显(P>0.05),治疗组显著降低(P<0.05),治疗后两组间比较,治疗组明显低于对照组(P<0.05);治疗组总有效率高于对照组(P<0.05);治疗期间两组均未出现药物相关不良事件报告。结论:萆薢泽兰饮联合洛索洛芬钠治疗急性痛风性关节炎湿热蕴结证患者可能通过减轻机体炎症反应,降低血尿酸水平,从而缓解受累关节临床症状,且短期使用安全性良好。Objective:To evaluate the clinical efficacy of Bixie Zelan decoction combined with loxoprofen sodium in the treatment of patients with acute gouty arthritis of hygrothermal syndrome.Methods:A total of 112 patients of acute gouty arthritis with hygrothermal syndrome were included,and they were randomly divided into treatment group and control group,56 cases in each group.The control group was orally administered loxoprofen sodium tablets on the basis of dietary and lifestyle adjustments,and the treatment group was orally administered Bixie Zelan decoction on the basis of the treatment of the control group,for a period of 7 days.Visual analogue scale(VAS),inflammatory factor levels[C-reactive protein(CRP),erythrocyte sedimentation rate(ESR)],blood uric acid(UA),musculoskeletal ultrasonography of the affected joints of the OMERACT consensus-based semi-quantitative ultrasonography gout scoring system score(ultrasonography score),Chinese medicine symptoms score,total effective rate,and adverse reactions were recorded before and after treatment.Results:VAS,inflammatory factor levels and Chinese medicine symptoms score were significantly improved in both groups after treatment,and the improvement in the treatment group was better than that in the control group(P<0.05).When comparing the UA and ultrasonography score,there was no significant reduction in the control group after treatment(P>0.05),while the UA and ultrasonography score decreased in treatment group(P<0.05).The treatment group showed lower UA and ultrasonography score than control group after treatment(P<0.05).The treatment group showed higher total effective rate than control group(P<0.05).There was no drug-related adverse event in both groups during the treatment period.Conclusion:The treatment of patients with acute gouty arthritis of the hygrothermal syndrome by Bixie Zelan decoction combined with loxoprofen sodium,may alleviate the clinical symptoms of the affected joints by reducing the inflammatory response of the body,reduce the level of blood uric ac
