地龙散联合中药热奄包治疗血瘀型腰椎间盘突出症的疗效及其对血清炎症因子水平的影响

摘　　要：目的:探讨地龙散联合中药热奄包治疗血瘀型腰椎间盘突出症的疗效及其对血清炎症因子水平的影响。方法:将100例血瘀型腰椎间盘突出症患者随机分为治疗组与对照组,每组50例。对照组患者予塞来昔布胶囊联合中药热奄包治疗,治疗组患者予地龙散联合中药热奄包治疗,7 d为1个疗程,治疗2个疗程。比较两组患者临床疗效,评价患者治疗前后疼痛视觉模拟评分法(VAS)评分及日本骨科学会腰椎评分(JOA评分),检测治疗前后患者白细胞计数(WBC)、C反应蛋白(CRP)、白介素-6(IL-6)及肿瘤坏死因子-α(TNF-α)。结果:对照组总有效率为78.00%(39/50),治疗组总有效率为98.00%(49/50),治疗组高于对照组(P<0.05);两组患者治疗后WBC、CRP、IL-6及TNF-α均低于治疗前(P<0.05),且治疗组患者治疗后WBC、CRP、IL-6及TNF-α低于对照组(P<0.05);两组患者治疗后VAS评分低于治疗前(P<0.05),JOA评分高于治疗前(P<0.05),且治疗组患者治疗后VAS评分低于对照组(P<0.05),JOA评分高于对照组(P<0.05)。结论:地龙散联合中药热奄包治疗血瘀型腰椎间盘突出症具有显著的临床疗效,且能降低炎症因子水平。Objective:To evaluate the efficacy of Dilong powder combined with traditional Chinese medicine hot compress comprehensive therapy for the blood stasis type lumbar disc herniation and its effects on serum pro-inflammatory cytokines levels.Methods:A total of 100 patients with blood stasis type lumbar disc herniation were randomly divided into two groups:control group(50 cases)and treatment group(50 cases).The control group received Celecoxib capsules combined with traditional Chinese medicine hot compress,while the treatment group received a combination of Dilong powder and traditional Chinese medicine hot compress therapy.Treatment was administered with each course lasting for 7 days,and two courses were conducted in total.The clinical efficacy of the two groups were compared,and the visual analogue scale(VAS)scores,Japanese Orthopaedic Association(JOA)scores,white blood cell(WBC),C-reactive protein(CRP),interleukin-6(IL-6),and tumor necrosis factor-alpha(TNF-α)levels of the patients before and after treatment were observed.Results:The total effective rate in the treatment group was 98.00%(49/50),while it was 78.00%(39/50)in the control group,the clinical efficacy of the treatment group was superior to that in the control group(P<0.05).After treatment,the WBC,CRP,IL-6,and TNF-αlevels of both groups were decreased(P<0.05),and the inflammatory cytokines levels of the treatment group significantly were lower than those of the control group(P<0.05).The VAS scores of both groups were dropped(P<0.05),and the JOA score were raised(P<0.05).The VAS scores of the treatment group were lower than those of the control group(P<0.05),while the JOA scores of the treatment group were higher than those of the control group(P<0.05).Conclusion:Combining Dilong powder with traditional Chinese medicine hot compress therapy can effectively improve clinical outcomes of blood stasis type lumbar disc herniation and reduce pro-inflammatory cytokines levels.
