摘 要:目的探讨疏血通联合辛伐他汀治疗慢性阻塞性肺疾病(COPD)合并肺动脉高压患者对内皮素-1(ET-1)、氨基末端B型利钠肽前体(NT-proBNP)表达水平的影响,为临床实践中评估此类患者治疗效果提供科学依据。方法将101例河南省南阳市中医院2021年5月至2023年6月收治的COPD合并肺动脉高压患者纳入本研究,按照治疗方法分为对照组(51例)、观察组(50例),对照组患者进行辛伐他汀治疗,观察组患者进行疏血通联合辛伐他汀治疗,评估对照组、观察组患者治疗前后凝血指标[凝血酶原时间(PT)、血浆凝血酶时间(TT)、纤维蛋白原(FIB)]、ET-1、NT-proBNP、一氧化氮(NO)、血液流变学[全血低切黏度(LS)、全血高切黏度(HS)、血浆黏度(PV)]指标表达水平。结果治疗后相较于治疗前对照组、观察组患者PT、TT延长,观察组相较于对照组更长(t=7.200、6.483,P<0.05);FIB水平降低,观察组相较于对照组更低(t=6.214,P<0.05)。治疗后相较于治疗前对照组、观察组患者ET-1、NT-proBNP水平降低,观察组相较于对照组更低(t=10.896、20.608,P<0.05);NO水平升高,观察组相较于对照组更高(t=1.949,P<0.05)。治疗后相较于治疗前对照组、观察组患者LS、HS、PV水平降低,观察组相较于对照组更低(t=6.705、5.180、6.612,P<0.05)。结论针对COPD合并肺动脉高压患者采用疏血通、辛伐他汀的联合用药方案,能够优化患者的凝血功能,调节ET-1、NT-proBNP、NO水平,缓解血液高凝状态,故凝血指标、ET-1、NT-proBNP、NO、血液流变学指标均可评估效果。Objective To investigate the effect of Shuxuetong combined with simvastatin on the expression levels of endothelin-1(ET-1)and N-terminal pro-B-type natriuretic peptide(NT-proBNP)in patients with chronic obstructive pulmonary disease(COPD)complicated with pulmonary hypertension,and to provide a scientific basis and reference framework for assessing the therapeutic efficacy of such patients in clinical practice.Methods A total of 101 patients with COPD complicated with pulmonary hypertension admitted to Nanyang Traditional Chinese Medicine Hospital from May 2021 to June 2023 were enrolled in this study.All patients were divided into a control group(51 patients)and an observation group(50 patients)based on different treatment approaches.The control group received simvastatin treatment,while the observation group received a combination of Shuxuetong and simvastatin.The expression levels of coagulation indicators[prothrombin time(PT),plasma thrombin time(TT),fibrinogen(FIB)],ET-1,NT-proBNP,nitric oxide(NO),and hemorheological indicators[low-shear whole blood viscosity(LS),high-shear whole blood viscosity(HS),plasma viscosity(PV)]before and after treatment were evaluated in the two groups.Results After treatment,compared with before treatment,the PT and TT of patients in the control group and observation group were prolonged,and the observation group was longer than the control group(t=7.200,6.483,P<0.05);The FIB level decreased,and the observation group had a lower level compared to the control group(t=6.214,P<0.05).After treatment,compared with before treatment,the levels of ET-1 and NT proBNP in the control group and observation group decreased,and the observation group was lower than the control group(t=10.896,20.608,P<0.05);The level of NO increased,and the observation group had a higher level compared to the control group(t=1.949,P<0.05).After treatment,compared with before treatment,the LS,HS,and PV levels of patients in the control group and observation group decreased,and the observation group had lower levels
