血清基质金属蛋白酶-9、核因子-κB、细胞外基质蛋白-3与Stanford B型主动脉夹层的相关性研究

摘  要:目的:探讨血清基质金属蛋白酶-9 (MMP-9)、核因子-κB (NF-κB)、细胞外基质蛋白-3 (Fibulin-3)与Stanford B型主动脉夹层患者预后关系及对预后的预测效能。方法:选取2020年12月至2022年12月我院收治的122例Stanford B型主动脉夹层患者为研究组,同期122名健康体检者为对照组,比较2组血清MMP-9、NF-κB、Fibulin-3水平。所有主动脉夹层患者均进行覆膜支架腔内修复术治疗,术后随访3个月,根据术后3个月后预后情况将患者分为预后不良(24例)和预后良好(98例)2个亚组。比较不同预后患者血清MMP-9、NF-κB、Fibulin-3水平;分析血清MMP-9、NF-κB、Fibulin-3与预后不良的相关性、影响因素以及联合检测价值。结果:研究组血清MMP-9、NF-κB水平高于对照组,Fibulin-3水平低于对照组,差异均有统计学意义(P<0.01);预后不良组术后7 d、14 d血清MMP-9、NF-κB水平高于预后良好组,Fibulin-3水平低于预后良好组,差异均有统计学意义(P<0.01);术后7 d、14 d血清MMP-9、NF-κB水平与预后不良均呈正相关,血清Fibulin-3水平与预后不良均呈负相关,差异均有统计学意义(P<0.05);术后7 d、14 d高水平的MMP-9、NF-κB是主动脉夹层患者预后不良的危险因素,术后7 d、14 d高水平的Fibulin-3是主动脉夹层患者预后不良的保护因素;受试者工作特征(ROC)曲线分析显示,术后7 d、14 d血清MMP-9、NF-κB、Fibulin-3水平联合预测主动脉夹层患者预后不良的AUC分别为0.738 (95%CI:0.676~0.806)、0.805 (95%CI:0.708~0.916)。结论:血清MMP-9、NF-κB、Fibulin-3与主动脉夹层患者病情恢复状况关系密切,可作为临床诊断、预后预测的有效指标。Objective:To investigate the relationship of serum matrix metalloproteinase-9(MMP-9),nuclear factor-κB(NF-κB),extracellular matrix protein-3(Fibulin-3)with prognosis of patients with Stanford type B aortic dissection and their predictive efficacy.Methods:A total of 122 patients with Stanford type B aortic dissection admitted to our hospital from Dec 2020 to Dec 2022 were selected as the research group,and 122 healthy physical examination subjects were selected as the control group.Serum levels of MMP-9,NF-κB,and Fibulin-3 were compared between the two groups.All patients with aortic dissection underwent endovascular stent graft repair,and were followed up for 3 months after surgery.According to the prognos is after 3 months,the patients were divided into 2 subgroups with poor prognosis(24 cases)and good prognosis(98 cases).The serum levels of MMP-9,NF-κB,and Fibulin-3 were compared between patients with different prognosis.The correlation of serum MMP-9,NF-κB,and Fibulin-3 with poor prognosis,influencing factors,and the value of combined detection were analyzed.Results:Serum MMP-9 and NF-κB levels were higher in the research group than those in the control group,and Fibulin-3 level was lower than that in the control group(P<0.01).Serum MMP-9 and NF-κB levels in the poor prognosis group were higher than those in the good prognosis group at 7 and 14 d postoperatively,and Fibulin-3 level was lower than that in the good prognosis group(P<0.01).Serum MMP-9 and NF-κB levels at 7 d and 14 d postoperatively were positively correlated with poor prognosis,and serum Fibulin-3 level was negatively correlated with poor prognosis(P<0.05).High levels of MMP-9 and NF-κB at 7 d and 14 d postoperatively were risk factors for poor prognosis in patients with aortic dissection,and high level of Fibulin-3 were protective factors for poor prognosis.Receiver operating characteristic(ROC)curve analysis showed that the AUC of serum MMP-9,NFκB,and Fibulin-3 levels combined to predict poor prognosis in patients with aortic dissect
