摘 要:目的比较西藏地区某医院LCBDE术后一期缝合与T管引流治疗胆总管结石疗效。方法回顾分析2022年4月至2024年4月西藏地区某医院普通外科胆囊结石合并胆总管结石并行LCBDE术105例患者临床资料,按照手术方式不同分为一期缝合组及T管引流组,其中一期缝合组63例,T管引流组42例。比较两种术式患者手术时间、术中出血、术后并发症、术后胃肠道恢复时间、术后指标恢复情况、术后整体恢复时间、住院时间以及住院费用等情况。结果在手术时间、术中出血、相对住院天数、住院费用、术后并发症方面一期缝合组均优于T管引流组(P<0.05),在术后恢复时间、指标恢复情况、胃肠道恢复时间方面两种术式无明显差异(P>0.05)。结论在LCBDE术后一期缝合处理胆总管手术时间更短,术中出血更少,住院时间更短,术后并发症更少,且更经济。Objective To compare the efficacy of primary suture versus T-tube drainage in laparoscopic cholecystectomy with common bile duct exploration(LCBDE)for treating common bile duct stones in a hospital in Xizang.Methods This study retrospectively analyzes the clinical data of 105 patients with gallstones and concurrent common bile duct stones who underwent LCBDE in the General Surgery Department of a hospital in Xizang from April,2022 to April,2024.Patients were divided into a primary suture group(63 cases)and a T-tube drainage group(42 cases)based on the surgical approach.This study compares the surgery time,intraoperative bleeding,postoperative complications,postoperative gastrointestinal recovery time,postoperative indicator recovery,overall postoperative recovery time,hospital stay,and hospitalization costs between the two groups.Results The primary suture group had shorter surgery time,less intraoperative bleeding,shorter hospital stay,lower hospitalization costs,and fewer postoperative complications than the T-tube drainage group(P<0.05).However,there was no significant difference in postoperative recovery time,indicator recovery,or gastrointestinal recovery time between the two groups(P>0.05).Conclusion Primary suture after LCBDE for the treatment of common bile duct stones results in shorter surgery time,less intraoperative bleeding,shorter hospital stay,fewer postoperative complications,and is more cost-effective compared to T-tube drainage.
