摘 要:目的探讨全身免疫炎症指数(systemic immune inflammation index,SII)预测儿童哮喘急性发作及其病情严重程度的价值。方法选取急性发作期的哮喘患儿92例作为哮喘组,根据患儿病情严重程度再分为轻度(n=30)、中度(n=33)及重度(n=29)3组,选取同期健康体检儿童65例作为对照组,收集哮喘组和对照组受检者的一般资料[年龄、性别及体质量指数(body mass index,BMI)];抽取哮喘组患儿治疗前和对照组儿童体检当日的外周静脉血,采用全自动血细胞分析仪检测中性粒细胞(neutrophil,Neu)、淋巴细胞(lymphocyte,Lym)及血小板(platelet,Plt)计数、并计算SII,采用流式免疫荧光法检测血清中的白细胞介素4(IL-4)和IL-6水平;采用肺功能仪检测哮喘组患儿治疗前和对照组儿童体检当日第1秒用力呼气容积占预计值百分比(FEV1%)和1秒用力呼气容积/用力肺活量(FEV1/FVC)数值;采用Spearman相关分析计算哮喘组患儿的SII水平与上述各指标间的相关性,绘制受试者工作特征(ROC)曲线下面积(AUC)评估哮喘组SII对哮喘急性发作及重度发作的预测价值。结果哮喘组患儿SII水平高于对照组(P<0.05),且哮喘组患儿表现为重度组>中度组>轻度组(P<0.05);哮喘组患儿SII与IL-4、IL-6呈正相关(P<0.05),与FEV1%、FEV1/FVC呈负相关(P<0.05);ROC结果显示SII预测哮喘急性发作、哮喘急性重度发作的AUC分别为0.825、0.797。结论SII对儿童哮喘急性发作及其病情严重程度评估有一定的预测价值。Objective To study the value of systemic immune inflammation index(SII)in predicting acute asthma attack and its severity in children.Methods A total of 92 children with acute asthma were selected as the asthma group.Based on the severity of the disease,they were further divided into mild group(n=30),moderate group(n=33),and severe group(n=29).Sixty-five healthy children in the same period were selected as the control group.The general data concerning age,gender,and body mass index(BMI)of the two groups were collected.Peripheral venous blood was collected from the children in the asthma group before treatment and from the children in the control group on the day of physical examination.Neutrophil(Neu),lymphocyte(Lym),and platelet(Plt)counts were detected by automatic blood cell analyzer,and SII was calculated.The levels of interleukin-4(IL-4)and interleukin-6(IL-6)in serum were detected by flow cytometry immunofluorescence method.The percentage of forced expiratory volume in one second(FEV1%)and forced expiratory volume in one second/forced vital capacity(FEV1/FVC)were measured by pulmonary function instrument from children in the asthma group before treatment and from children in the control group on the day of physical examination.Spearman correlation analysis was used to analyze the correlation between SII level and the above-mentioned indicators in the asthma group.The area under the receiver operating characteristic(ROC)curve was drawn and area under the curve(AUC)was adopted to examine the predictive value of SII in the asthma group for acute and severe asthma attacks.Results The level of SII in asthma group was significantly higher than that in control group(P<0.05),and that level of severe group was higher than that of moderate group while the moderate group was higher than mild group(P<0.05).SII in asthma group was positively correlated with IL-4 and IL-6(P<0.05)and was negatively correlated with FEV1%and FEV1/FVC(P<0.05).ROC results showed that the AUC of SII in predicting acute asthma attack and acute
