摘 要:目的探讨宽体探测器轴扫描联合迭代重建技术剂量控制新功能在患儿童胸部CT检查低辐射剂量扫描中的应用价值。方法选取120例需要进行胸部CT的学龄前儿童。通过使用随机数字表,将患儿随机分为A、B、C、D四组。A组作为对照组,ASiR-V权重设置为0(FBP),B、C、D组作为实验组,ASiR-V权重分别设置为20%、40%和60%。四组均采用16 cm探测器轴扫描,管电压100 kVp,自动管电流设置范围10~600 mA,球管旋转速度0.28 s/rot,噪声指数为12,层厚5 mm。记录每例患儿检查后所得的剂量长度乘积(dose length product,DLP)值,测量每例患儿纵隔窗主动脉的噪声(SD)值,通过单因素方差分析,比较组间DLP、SD的差异,并采用双盲法对肺窗及纵隔窗进行5分制主观图像质量评价。结果四组的DLP值分别为(30.16±13.75)、(21.33±8.19)、(16.41±6.66)、(10.26±4.60)mGy·cm,DLP值在四组的组间差异均有统计学意义(P<0.05),与ASiR-V为0(FBP)的图像比较,随着ASiR-V升高,DLP分别降低29.28%、45.59%、65.98%。SD值分别为(9.92±1.31)HU、(9.79±1.21)HU、(9.55±1.34)HU、(9.47±0.85)HU,SD值在四组的组间差异均无统计学意义(P>0.05)。结论在学龄前儿童的胸部CT检查中,利用迭代重建剂量控制新功能,ASiR-V权重改变不影响图像质量,还可显著降低辐射剂量。objective This research intended to investigate the application value of iterative reconstruction(ASiR-V)combined with axial scanning and wide body detector in dose control for low-dose children CT scanning.Methods 120 preschool children were prospectively enrolled in this study.The patients were randomly divided into four groups.The control group was set to group A,the ASiR-V weight was set to 0%(FBP),the experimental group was set to group B,C,D,and the ASiR-V weight was set to 20%,40%and 60%respectively.The children in four groups were scanned by 16 cm detector axis,voltage 100 kVp,automatic tube current setting range 10-600 mA,rotation speed 0.28 s/rot,noise index 12,and scanning layer thickness 5 mm.Dose Length Product(DLP)values were recorded,and the standard deviation(SD,image noise)values of each patient's image were measured with the mediastinal window aorta.The differences of DLP and SD were compared by single factor ANOVA.Subjective image quality was evaluated blindly using a 5-point scoring method.Results The DLP values of group A,B,C,D were(30.16±13.75),(21.33±8.19),(16.41±6.66),(10.26±4.60)mGy·cm,and the SD values were(9.92±1.31)HU,(9.79±1.21)HU,(9.55±1.34)HU,(9.47±0.85)HU.There were significant differences in DLP between any two groups(P<0.05).Compared with imaging of ASiR-V 0,as the increase of ASiR-V,the reduction percentage of DLP was 29.28%,45.59%and 65.98%,respectively.There were no significant differences in SD and subjective score between any two groups(P>0.05).Conclusion The new function of iterative reconstruction dose control was used in chest CT examination of preschool children.ASiRV weight change did not affect image quality(SD),but the radiation dose could be significantly reduced.
