温经通络外敷方治疗膝关节创伤性滑膜炎的临床效果

摘　　要：目的 评价温经通络外敷方对基层军事训练人员膝关节创伤性滑膜炎的临床效果。方法 选取104例确诊为膝关节创伤性滑膜炎的军事训练人员,随机分为试验组和对照组,每组52例,试验组在股四头肌功能锻炼的基础上给予温经通络外敷方治疗,对照组在股四头肌功能锻炼的同时外用双氯芬酸二乙胺软膏透皮吸收治疗,两组疗程均为7天,治疗后应用膝关节视觉模拟评分(visual analogue scale, VAS)评估膝关节疼痛的缓解情况、Lysholm评分评估膝关节功能的恢复情况、膝关节积液评分评估关节腔积液的改善程度。结果 治疗后,两组军事训练人员VAS评分均明显下降,且试验组低于对照组(P均<0.001);两组Lysholm评分均明显增加,且试验组优于对照组(P均<0.001);试验组膝关节积液评分明显低于治疗前,且评分明显低于对照组(P<0.001),但是对照组膝关节积液评分治疗前后比较差异无统计学意义(P>0.05)。结论 温经通络外敷方可明显缓解膝关节创伤性滑膜炎引起的疼痛、肿胀、活动不利等症状,改善膝关节功能,在促进膝关节积液吸收方面具有明确优势,且使用方法简单,尤其适合诊疗不便的基层军事训练人员。Objective To explore the clinical efficacy of WenJing Tongluo external prescription on traumatic synovitis of knee joint of military training personnels.Methods A total of 104 military training personnels who diagnosed with traumatic synovitis of knee joint were randomly divided into experimental group and control group,with 52 personnels in each.The experimental group was given WenJing Tongluo external prescription on the basis of quadriceps functional exercise,and the control group was given Diclofenac Diethylamine Emulgel on the basis of quadriceps functional exercise.The course of treatment was 7 d,after which the relief of knee pain was assessed by visual analogue scale(VAS),recovery of knee function was assessed by Lysholm scores and improvement of joint effusion was assessed by knee joint effusion scores.Results After the treatment,the VAS scores of the two groups of military training personnels decreased significantly,and the score in experimental group decreased significantly than that in control group(all P<0.001);the Lysholm scores of the two groups of military training personnels increased significantly,and the score in experimental group was significant higher than that in control group(all P<0.001);the knee joint effusion scores of military training personnels in the experimental group significant decreased than that before treatment,and the score was significant lower than that of the control group(P<0.001),however,there was no significant difference in the knee joint effusion scores of the control group before and after treatment(P>0.05).Conclusion WenJing Tongluo external prescription can significantly alleviate the pain,swelling,adverse activity and other symptoms caused by traumatic synovitis of the knee joint,improve the function of the knee joint,and has clear advantages in promoting the absorption of knee joint effusion,and the use of simple methods,especially suitable for grassroots training personnels with inconvenient diagnosis and treatment.
