带线锚钉修复跖板撕裂的临床研究

摘　　要：目的探讨带线锚钉用于修复跖板撕裂的临床疗效。方法回顾性分析2016年3月至2019年3月河北省唐山市第二医院治的34例(34足)跖板撕裂患者临床资料,34例跖板损伤均发生在第2跖趾关节,术中见第2跖趾关节跖板撕裂,术中利用第2跖骨头Weil截骨,应用带线锚钉修复撕裂跖板,观察分析治疗效果。结果术后患者手术切口均达到I期愈合。34例患者均获得24~30个月随访,平均25.8个月。术后足趾外观畸形均得到纠正。术后24个月依据美国足踝外科协会(American Orthopedic Foot and Ankle Society,AOFAS)评分(90.20±3.99)分,显著高于术前的(55.60±3.86)分,差异有统计学意义(t=32.438,P=0.000)。VAS疼痛评分:术后24个月(0.70±1.06)分,显著低于术前的(7.70±0.95)分,差异有统计学意义(t=19.170,P=0.000)。结论跖板对于保证跖趾关节的稳定至关重要,应用带线锚钉修复跖板撕裂可以获得较好的近期效果。Objective To investigate the clinical effect of suture anchor in repairing the plantar platetear.Methods The clinical data of 34 patients(34 feet)with admitted plantar platetears to the foot and ankle surgery department of he Second Hospital of Tangshan from March 2016 to March 2019 were retrospectively analyzed.34 cases all occurred in the second metatarsophalangeal joint.Intraoperative plantar platetearwas observed at the second metatarsophalangeal joint.Weil osteotomy was used in the second metatarsal bone and suture anchors were used to repair the torn plantar plate,and observe and analyze the therapeutic effect.Results All the incisions were healed in stage I.All the 34 patients were followed up for 24 to 30 months with an average of 25.8 months.All the deformities were corrected.According to the American Orthopedic Foot and Ankle Society(AOFAS)standard score at 24 months after operation(90.20±3.99)points was significantly higher than that before operation(55.60±3.86)points,and the difference was statistically significant(t=32.438,P=0.000).VAS pain score:24 months after operation(0.70±1.06)points,significantly lower than that before operation(7.70±0.95)points,the difference was statistically significant(t=19.170,P=0.000).Conclusion The plantar plate play a crucial role in the stability metatarsophalangeal joint.The application of suture anchor to repair the plantar platetears can obtain satisfactory short-term results.
