系统康复对慢性踝关节不稳患者全关节镜术后关节功能的疗效分析

摘　　要：目的研究系统康复对全关节镜下治疗慢性踝关节不稳患者术后功能恢复的效果。方法将武警江苏总队医院2021年6月至2023年6月收治的在全关节镜下行踝关节外侧副韧带修复手术的36例患者按治疗方式分为系统康复组和对照组,其中系统康复组18例,对照组18例。两组患者术后均采用常规治疗,系统康复组同时进行系统康复训练。术后1周、4周和12周对两组患者进行视觉模拟评分法(visual analogue scale,VAS)评分评估;采用美国足踝外科协会(American Orthopaedic Foot and Ankle Society,AOFAS)踝-后足评分在术后4周、12周和24周进行踝关节功能评估。结果系统康复组术后4周、12周的VAS评分均低于对照组,差异有统计学意义(P<0.05);系统康复组患者术后4周、12周、24周AOFAS评分较对照组有显著提高(P<0.05)。结论慢性踝关节不稳定的患者全关节镜下手术治疗后,经系统康复的踝关节功能较一般治疗恢复更好。Objective To study the effect of systematic rehabilitation on functional recovery of patients with chronic ankle instability after total arthroscopy.Methods 36 patients who underwent arthroscopic repair surgery for the lateral collateral ligament of the ankle joint at the Jiangsu General Hospital of the People’s Armed Police Force from June 2021 to June 2023 were divided into a systematic rehabilitation group and a control group according to the treatment method,with 18 patients in the systematic rehabilitation group and 18 patients in the control group.Patients in both groups were treated with routine rehabilitation after operation,and the rehabilitation group was also given systematic rehabilitation training.Visual analog scale(VAS)of patients in the two groups was observed at 1 week,4 weeks and 12 weeks after operation.The ankle function of the patients in the two groups was evaluated at 4 weeks,12 weeks and 24 weeks after operation,and the ankle-hindfoot score of American Orthopaedic Foot and Ankle Society(AOFAS)was used and compared.Results The VAS scores of the system rehabilitation group at 4 weeks postoperatively and at 12 weeks postoperatively were both lower than those of the control group,with statistically significant differences(P<0.05).The AOFAS scores in the systematic rehabilitation group at 4 weeks,12 weeks and 24 weeks after surgery were significantly higher than those in the control group(P<0.05).Conclusion In patients with chronic ankle instability,ankle function recovered better after systemic rehabilitation under total arthroscopic surgery than conventional treatment.
