全程管理模式在行微创踝关节融合术治疗的青年创伤性踝关节炎患者中的效果分析

摘　　要：目的探讨全程管理模式在行微创踝关节融合术治疗青年创伤性踝关节炎患者中的效果分析。方法东南大学附属中大医院2019年1月至2022年12月接收156例微行创踝关节融合术治疗的青年创伤性踝关节炎患者,按随机数表法分为研究组(在常规护理的基础上实施全程管理模式)及对照组(行常规护理)两组,每组各78例。分析两组患者的心理状态、疼痛程度、踝关节活动度、骨代谢指标及不良反应发生情况。结果干预后,研究组的焦虑自评量表、抑郁自评量表低于对照组(P<0.05);干预后,研究组疼痛程度低于对照组(P<0.05);干预后,研究组的踝关节活动度高于对照组(P<0.05);研究组血清碱性磷酸酶、降钙素、骨钙素高于对照组,而血清酸性磷酸酶、甲状旁腺激素低于对照组(P<0.05);研究组不良反应总发生率为1.28%,低于对照组的10.26%(P<0.05)。结论全程管理模式应用在行微创踝关节融合术在治疗的青年创伤性踝关节炎患者时,能够显著改善患者的心理状态,减轻术后疼痛,并有效提高患者的关节活动度,优化骨代谢指标。Objective To explore the effect of the whole management mode in minimally invasive ankle joint fusion in the treatment of young patients with traumatic ankle arthritis.Methods From January 2019 to December 2022,a total of 156 young patients with traumatic ankle arthritis treated by minimally invasive ankle joint fusion were received in Zhongda Hospital Affiliated to Southeast University,and were divided into study group(complete management mode based on routine care)and control group(routine care)according to random number table method,with 78 cases in each group.The psychological state,pain degree,ankle range of motion,bone metabolism index and the occurrence of adverse reactions were analyzed.Results After intervention,the self-rating anxiety scale and self-rating depression scale of the study group were lower than those of the control group(P<0.05).After intervention,the pain degree of the study group was decreased compared with the control group(P<0.05).After intervention,the ankle joint motion of the study group was higher than that of the control group(P<0.05).Serum alkaline phosphatase,calcitonin and osteocalcin in the study group were higher than those in the control group,but serum acid phosphatase and parathyroid hormone were lower than those in the control group(P<0.05).The total incidence of adverse reactions in the study group was 1.28%,which was lower than that in the control group(10.26%)(P<0.05).Conclusion In the treatment of young patients with traumatic ankle arthritis,total management mode combined with minimally invasive ankle joint fusion can significantly improve patients’psychological state,reduce postoperative pain,effectively improve patients’joint range of motion,and optimize bone metabolism indexes.
