肺恶性肿瘤患者住院费用的结构变动度和新灰色关联分析

摘　　要：目的分析洛阳某三甲医院肺恶性肿瘤患者住院费用,了解费用结构变化特征和内部因素关联程度,为该病种费用的优化提供案例研究和参考。方法收集2019~2022年洛阳某三甲医院肺恶性肿瘤患者的住院费用资料,采用结构变动度和新灰色关联两种方法进行分析。结果该院肺恶性肿瘤患者治疗费结构变动贡献率(36.64%)最大,药品费和护理费为2.05%和3.71%;费用关联度前三位为药品费(0.998)、治疗费(0.977)、手术费(0.947)。结论该院肺恶性肿瘤患者费用结构有待优化,药品费和护理费的结构变动贡献率有待提高,药品费是影响费用的首要因素,建议深化药品集采和推进支付方式改革,进一步优化费用结构和降低住院费用。OBJECTIVE To analyze the hospitalization expenses of patients with lung cancer in a tertiary hospital in Luoyang,to understand the characteristics of the cost structure and the degree of internal factors,and to provide a case study and reference for the optimization of the cost of this disease.METHODS The data of hospitalization expenses of patients with lung cancer in a tertiary hospital in Luoyang from 2019 to 2022 were collected,and analyzed by two methods:structural variation degree and new grey correlation.RESULTS The treatment cost structure of patients with lung cancer in the hospital contributed the most to the change(36.64%),and the drug cost and nursing cost were 2.05% and 3.71%,respectively.The top three related degrees of expenses were drug cost(0.998),treatment cost(0.977),and surgery cost(0.947).CONCLUSION The cost structure of patients with lung malignant tumor in the hospital needs to be optimized,and the contribution rate of the structural changes of drug cost and nursing cost needs to be improved.Drug cost is the primary factor affecting the cost.It is recommended to deepen the centralized drug collection and promote the reform of payment method,further optimize the cost structure and reduce hospitalization costs.
