解毒敛疮膏联合封闭式负压引流技术治疗深Ⅱ度烧伤创面的临床研究

摘　　要：目的:探讨解毒敛疮膏联合封闭式负压引流(VSD)技术治疗深Ⅱ度烧伤创面的临床疗效。方法:前瞻性选取2022年1月~2024年1月期间我院收治的108例深Ⅱ度烧伤创面患者,采用随机双色球法将患者分为对照组和研究组,每组各54例。对照组接受VSD技术治疗,研究组接受解毒敛疮膏联合VSD技术治疗。对比两组临床总有效率、创面完全愈合时间、肉芽生长时间、创面渗液干净时间、创面愈合率、炎症因子水平[降钙素原(PCT)、白细胞介素-8(IL-8)、白细胞介素-6(IL-6)、干扰素γ(IFN-γ)]及瘢痕情况。结果:相较于对照组,研究组的临床总有效率更高,且创面完全愈合时间、肉芽生长时间、创面渗液干净时间均更短(P<0.05)。研究组治疗第7 d、治疗第14 d、治疗第21 d的创面愈合率均高于对照组(P<0.05)。研究组治疗7 d后PCT、IL-6、IL-8、IFN-γ水平均低于对照组(P<0.05)。研究组随访3个月后、随访6个月后的VSS评分均低于对照组(P<0.05)。结论:解毒敛疮膏联合VSD技术治疗深Ⅱ度烧伤创面,疗效显著,可有效促进创面愈合,改善创面癫痕情况,并降低炎症因子的表达水平。Objective:To investigate the clinical efficacy of Jiedu Lianchuang ointment combined with vacuum sealing drainage(VSD)technique in the treatment of deep Ⅱ degree burn wounds.Methods:108deepⅡdegree burn wounds patients who were admitted in our hospital from January 2022 to January2024 were prospectively selected,patients were divided into control group and study group by random two-color ball method,with 54 cases in each group.The control group was treated with VSD technology,and the study group was treated with Jiedu Lianchuang ointment combined with VSD technique.The total clinical effective rate,wound complete healing time,granulation growth time,wound exudate clean time,wound healing rate,inflammatory factors level[procalcitonin(PCT),interleukin-8(IL-8),interleukin-6(IL-6),interferon gamma(IFN-γ)]and scar condition were compared between the two groups.Results:Compared with control group,the total clinical effective rate in study group was higher,the complete wound healing time,granulation growth time and wound exudate clean time were shorter(P<0.05).the wound healing rates in study group were higher than in control group at 7 d,14 d and 21 d after treatment(P<0.05).The PCT,IL-6,IL-8 and IFN-γ levels in the study group were lower than those of the control group at 7 d after treatment(P<0.05).The VSS scores in study group were lower than those in control group 3 months after follow-up and 6 months after follow-up(P<0.05).Conclusion:Jiedu Lianchuang ointment combined with VSD technique in the treatment of deep Ⅱ degree burn wounds has a significant effect,which can effectively promote wound healing,improve wound scarring,and reduce the inflammatory factors expression level.
