复方甘草酸苷片联合耳后注射地塞米松治疗老年突发性耳聋的随机对照研究

摘　　要：目的:基于随机对照试验探讨耳后注射地塞米松、复方甘草酸苷片联合治疗老年突发性耳聋的临床疗效。方法:采用信封抽签法将2022年2月至2024年2月期间我院收治的101例老年突发性耳聋患者分为对照组(耳后注射地塞米松治疗,n=50)和研究组(复方甘草酸苷片联合耳后注射地塞米松治疗,n=51)。对比两组临床症状改善时间、不同频率听力阈值、血液流变学指标[全血低切黏度(LBV)、血浆黏度(PV)、纤维蛋白原(FIB)、全血高切黏度(HBV)]、炎症因子指标[超敏C反应蛋白(hs-CRP)、白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、白细胞介素-10(IL-10)]及不良反应发生情况。结果:研究组的耳鸣消失时间、眩晕消失时间、听力恢复时间均短于对照组(P<0.05)。两组治疗后0.5KHz、1KHz、2KHz听力阈值、PV、LBV、HBV、FIB、hs-CRP、IL-6、TNF-α、IL-10水平下降,且研究组低于对照组(P<0.05)。两组不良反应总发生率比较未见差异(P>0.05)。结论:老年突发性耳聋在耳后注射地塞米松治疗的基础上结合复方甘草酸苷片,可有效缩短临床症状改善时间,改善不同频率听力阈值、炎症指标及血液流变学指标,且具有较高的安全性。Objective:To investigate the clinical efficacy of retroauricular injection of dexamethasone combined with compound glycyrrhizin tablets in the treatment of elderly with sudden deafness base on randomized controlled trials.Methods:101 elderly patients with sudden deafness who were admitted to our hospital from February 2022 to February 2024 were selected.The patients were randomly divided into control group(treated with retroauricular injection of dexamethasone,n=50)and study group(treated with compound glycyrrhizin tablets combined with retroauricular injection of dexamethasone,n=51)by envelope drawing method.The improvement time of clinical symptoms,hearing threshold at different frequencies,hemorheological indexes[whole blood low shear viscosity(LBV),plasma viscosity(PV),fibrinogen(FIB),whole blood high shear viscosity(HBV)],inflammatory factor indexes[high-sensitivity C-reactive protein(hs-CRP),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),interleukin-10(IL-10)]and adverse reactions were compared between two groups.Results:The tinnitus disappearance time,vertigo disappearance time and hearing recovery time in study group were shorter than those in control group(P<0.05).The hearing thresholds of 0.5 KHz,1 KHz and 2 KHz,PV,LBV,HBV,FIB,hs-CRP,IL-6,TNF-α and IL-10 decreased after treatment,and study group were lower than those of control group(P<0.05).There was no significant difference in the total incidence of adverse reactions between two groups(P>0.05).Conclusion:The treatment of sudden deafness in the elderly was based on retroaural injection of dexamethasone combined with compound glycyrrhizin tablets,which can effectively shorten the improvement time of clinical symptoms,improve hearing threshold at different frequencies,inflammatory indexes and hemorheological indexes,and has high safety.
