上海市某三甲医院2019-2023年抗抑郁药使用分析

摘　　要：目的:了解上海市某三级甲等综合性医院抗抑郁药的使用情况,为促进临床合理用药提供参考。方法:采用WHO推荐的规定日剂量分析法,对上海市某三甲医院2019—2023年抗抑郁药的销售金额、主要品种、用药频度和日均费用及排序比等进行统计分析。结果:该院2019—2022年抗抑郁药销售额及其占所有西药药物销售金额的比例均呈下降趋势,2023年转为增长,且均显著高于同类医院。2019—2023年,该院抗抑郁药的规定日剂量数呈持续上升趋势,提示抗抑郁药物的临床使用频率增多;艾司西酞普兰以及文拉法辛的销售金额较高,占比维持在前2位,艾司西酞普兰的销售金额占比涨幅较大;帕罗西汀、艾司西酞普兰以及文拉法辛的规定日剂量数均位列前3。结论:该院抗抑郁药的临床使用结构较为合理,选择性5-羟色胺再摄取抑制剂与选择性5-羟色胺和去甲肾上腺素再摄取抑制剂两类药物占据主导地位。Objective:To investigate the usage of antidepressants in a tertiary comprehensive hospital in Shanghai and provide reference for rational drug use in clinical practice.Methods:Using the defined daily dose(DDD)analysis method recommended by WHO,we statistically analyzed the sales,major varieties,DDDs,ranking ratio,average daily cost,and defined daily doses of antidepressants in this hospital from 2019 to 2023.Results:From 2019 to 2022,the sales proportion of antidepressants compared to all Western medicines showed a downward trend,however,it turned into an increase in 2023 and remained significantly higher than the other similar hospitals.From 2019 to 2023,the DDDs for antidepressants in this hospital showed a continuous upward trend,suggesting an increasing clinical use frequency.Escitalopram and venlafaxine had high sales,maintaining top 2 rankings,with escitalopram showing the most significant increase.Meanwhile,paroxetine,escitalopram and venlafaxine consistently ranked among the top three in terms of DDD.Conclusion:The clinical use structure of antidepressants in this hospital is reasonable with selective serotonin reuptake inhibitors and selective serotonin-norepinephrine reuptake inhibitors dominating as major drug categories.
