摘　　要：目的探讨过敏性紫癜(Henoch-Schonlein purpura,HSP)患儿免疫功能变化及其临床意义。方法选取2021年1月至2022年12月南通大学附属常州市儿童医院收治的53例HSP患儿作为研究对象,根据症状积分分为轻症组(n=20)、中症组(n=22)与重症组(n=11),另选取同期53名健康儿童作为健康组。4组均检测IgA、IgG、IgM、C3、C4及外周血淋巴细胞亚群[CD3^(+)、CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)、B淋巴细胞、自然杀伤(nature killer,NK)细胞]。比较4组免疫球蛋白、补体及淋巴细胞亚群水平,分析病情严重程度与免疫球蛋白、补体及淋巴细胞亚群水平的相关性。结果IgA、IgG水平比较健康组<轻症组<中症组<重症组,C3水平比较健康组>轻症组>中症组>重症组,差异有统计学意义(P<0.05);CD8^(+)、B淋巴细胞水平比较健康组<轻症组<中症组<重症组,CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)、NK细胞水平比较健康组>轻症组>中症组>重症组,差异有统计学意义(P<0.05)。Pearson相关性分析显示,IgA、IgG、CD8^(+)、B淋巴细胞与病情严重程度呈正相关(r>0,P<0.05),补体C3、CD3^(+)、CD4^(+)、NK细胞及CD4^(+)/CD8^(+)与病情严重程度呈负相关(r<0,P<0.05)。结论HSP患儿免疫球蛋白、补体及淋巴细胞亚群水平紊乱,且与病情轻重程度相关,临床可通过检测外周血淋巴细胞亚群、免疫球蛋白及补体指标,分析患儿免疫功能变化,为HSP的诊断、治疗及预后评估提供重要依据。Objective To investigate the changes of immune function in children with Henoch-Schönlein purpura(HSP)and its clinical significance.Methods 53 children with HSP admitted to Changzhou Children's Hospital Affiliated to Nantong University from January 2021 to December 2022 were selected as the research subjects,they were divided into the mild group(n=20),the moderate group(n=22)and the severe group(n=11)ac-cording to the symptom score,53 healthy children in the same period were selected as the healthy group.IgA,IgG,IgM,C3,C4 and peripheral blood lymphocyte subsets(CD3^(+),CD4^(+),CD8^(+),CD4^(+)/CD8^(+),B lymphocytes,nature killer[NK]cells)were detected among the four groups.The levels of im-munoglobulin,complement and lymphocyte subsets among the four groups were compared,and the correlation between the severity of the disease and the levels of immunoglobulin,complement and lymphocyte subsets was analyzed.Results The levels of IgA and IgG were compared with those of the healthy group<the mild group<the moderate group<the severe disease group;the levels of C3 were compared with those of healthy group>the mild disease group>the moderate disease group>the severe disease group,and the differences were statistically significant(P<0.05).CD8^(+)and B lymphocyte levels were compared those of the healthy group<the mild group<the moderate group<the severe group;CD3^(+),CD4^(+),CD4^(+)/CD8^(+),nature killer(NK)cell levels were compared those of the healthy group>the mild group>the moderate group>the severe group,and the differences were statistically significant(P<0.05).Pearson correlation analysis showed that IgA,IgG,CD8^(+)and B lymphocytes were positively correlated with the severity of the disease(r>0,P<0.05),and complement C3,CD3^(+),CD4^(+),NK cells and CD4^(+)/CD8^(+)were negatively correlated with the severity of the disease(r<0,P<0.05).Conclusion The levels of immunoglobulin,complement and lymphocyte subsets in chil-dren with HSP are disordered,and are related to the severity of the disease,the changes of immune fun
