基于肠道菌群探讨清解二阳法缓解2型糖尿病进展的思路

摘　　要：肠道菌群失调是引起2型糖尿病的重要因素,肠道微生态平衡成为缓解2型糖尿病进展的重要切入点。嗜食肥甘导致胃肠功能失调,消化系统负担加重,脾胃运化能力减弱,进而中焦阻滞,气血运行障碍,内热蕴积,久积化火,故阳明胃肠积热、少阳肝胆郁热为2型糖尿病的核心病机,这与现代研究所发现的,在高糖高脂影响下菌群结构失衡,致炎菌比例增加,进而导致糖尿病发病密切相关。临证以清解少阳、阳明为代表的中药方剂,通过对肠道菌群的调控从而修复损坏的肠黏膜,降低炎症水平,改善胰岛素抵抗,促进胰岛功能恢复。Dysbiosis of intestinal flora is an important factor causing type 2 diabetes mellitus,and intestinal microecological balance has become an important entry point to alleviate the progression of type 2 diabetes mellitus.Eating too much fat and sweet food leads to gastrointestinal dysfunction,increased burden on the digestive system,and weak the Spleen and Stomach′s ability of transportation and digestion,and then cause Middle Jiao blockage,Qi and blood operation obstacles,internal Heat accumulation,accumulation of Fire eventually.Therefore,Yangming gastrointestinal Heat and shaoyang Liver and gallbladder Heat are the core pathogenesis of type 2 diabetes.This is closely related to the modern research that the imbalance of the bacterial flora structure under the influence of high sugar and high fat increases the proportion of inflammatory bacteria,which in turn leads to the development of diabetes mellitus.In clinical treatment,the prescriptions functioning in clearing Heat of Shaoyang and Yangming are used to repair the damaged intestinal mucosa,reduce the level of inflammation,improve insulin resistance,and promote the recovery of pancreatic islet function through the regulation of intestinal flora.
