摘　　要：IgA肾病以肾小球系膜区免疫球蛋白A沉积伴系膜增生为主要病理特点,部分患者初起可表现为急进性肾炎或肾病综合征,极易进展至慢性肾衰竭。IgA肾病的病理改变属中医癥瘕积聚范畴。《黄帝内经》提出:“积之始生,得寒乃生,厥乃成积也。”认为癥瘕积聚的形成始于肾阳虚衰,阴寒内生,继发气血运行障碍,导致瘀血和痰饮等有形病理产物的形成和裹结。基于此,在临床实践中运用桂枝茯苓丸以温阳化瘀、化气行水,可达到缓消癥积的治疗效果,有助于改善和延缓IgA肾病患者的肾脏病理进展。IgA nephropathy is characterized by immunoglobulin A deposition in the glomerular mesangial area with mesangial hyperplasia,and some of the patients may have acute nephritis or nephrotic syndrome at the beginning of the disease,and it is very easy to progress to chronic renal failure.IgA nephropathy belongs to the category of abdomen mass in traditional Chinese medicine.In Huang Di Nei Jing,it is suggested that"accumulation beginning with Cold injury and existing with injury progression".It is believed that the formation of mass in the abdomen begins with the weakening of Kidney Yang and the birth of inner Cold,which is followed by impaired Qi and blood circulation,resulting in the formation of tangible products of pathologies such as stasis of blood and Phlegm.Based on this,Guizhi Fuling Pill is used in clinical practice to warm Yang,eliminate stasis,resolve Qi and move Water to achieve the therapeutic effect of slowing down and elimination of mass accumulation,which can help to relieve and delay the progression of renal pathology in patients with IgA nephropathy.
