摘　　要：皮层下动脉硬化性脑病为本虚标实之证,病势缠绵,以虚实夹杂者为著。其证以肾虚为本,髓亏、肝郁为标,其病位在脑,多由于年老肾虚及情志不遂引起脑萎髓空,脑神及脏腑功能失调,故见神智减退、反应呆钝,兼见情志活动异常。肾虚精亏是病机关键所在,该病皆由此作,并贯穿疾病始终。除肾虚外,肝郁气滞亦不容轻视。临证当谨守病机,把握主线,治疗上应益肾疏肝,补脑充髓,使肝气得疏,脑髓得充,神机得俱。Subcortical arteriosclerotic encephalopathy belongs to syndrome of deficiency in root yet excess in symptom.As a lingering disease,it is most typical when there is a mixed case of deficiency and excess.Kidney deficiency is the root while marrow loss and Liver depression are symptoms.The disease is in the brain.The disease is mostly due to aging,Kidney deficiency and emotional disorders,which causes the brain atrophy and marrow empty,brain,spiritual disorder and Zang-fu dysfunction,so the symptoms such as intelligence loss,dull reaction,and abnormal emotional activities can be seen.Kidney deficiency and Essence loss is the key disease mechanism throughout the whole process of the disease.In addition to Kidney deficiency,Liver depression and Qi stagnation should not be neglected.In clinical practice,the disease mechanism and the main issue should be kept in mind.The treatment should be to benefit the Kidney and dredge the Liver,to replenish the brain and marrow,so that the Liver Qi can be soothed,the brain and marrow can be filled,and the spiritual condition can be normalized.
