雷公藤及其提取物调控相关信号通路治疗类风湿关节炎的机制研究进展

摘  要:雷公藤及其提取物作为临床上治疗类风湿关节炎的常规用药,其疗效得到了医学界的公认。现代药理研究表明,雷公藤及其雷公藤多苷、雷公藤甲素、雷公藤红素、雷藤舒等提取物,能干预并调控核因子-κB(nuclear factor-kappa B,NF-κB)、Janus激酶/信号转导与转录激活子(Janus kinase/signal transducer and activator of transcription,JAK/STAT)、丝裂原活化蛋白激酶(mitogen-activated protein kinases,MAPKs)、磷脂酰肌醇3-激酶(phosphatidylinositol 3-kinase,PI3K)蛋白激酶B(protein kinase B,AKT)、核因子-κB受体活化因子配体(receptor activator of NF-κB ligand,RANKL)/核因子-κB受体活化因子(receptor activator of nuclear factor-κB,RANK)/骨保护素(osteoprotegerin,OPG)、琥珀酸/血管内皮生长因子(vascular endothelial growth factor,VEGF)、cGAS-STING、胆碱能抗炎通路等信号通路,从而抑制关节炎症,减少血管生成,延缓骨破坏,达到治疗类风湿关节炎的目的。但目前不同信号通路之间相互影响的机制以及雷公藤相关复方调控信号通路的机制尚未阐明,亟须探讨雷公藤相关制剂对类风湿关节炎相关信号通路之间的疗效机制,进一步挖掘雷公藤的潜在价值,以期为雷公藤治疗类风湿关节炎的研究提供一定参考。The therapeutic efficacy of Leigongteng(Tripterygium wilfordii)and its extracts,as conventional drugs for the clinical treatment of rheumatoid arthritis,has been recognized by the medical community.Modern pharmacological studies have shown that Leigongteng(Tripterygium wilfordii)and its extracts such as Tripterygium glycosides,Triptolide,Celastrol and LLDT-8 can intervene in and regulate nuclear factor-κB(NF-κB),Janus kinase/signal transducer and activator of transcription(JAK/STAT),mitogen-activated protein kinases(MAPKs),phosphatidylinositol 3-kinase(PI3K)/protein kinase B(AKT),nuclear factor-κB receptor activator of NF-κB ligand(RANKL)/receptor activator of nuclear factor-κB(RANK)/osteoprotegerin(OPG),succinate/vascular endothelial growth factor(VEGF),cGAS-STING,cholinergic anti-inflammatory pathway,etc.,thus inhibiting joint inflammation,reducing angiogenesis,delaying bone destruction,and achieving the therapeutic effect of rheumatoid arthritis.However,the mechanism of interaction between different signaling pathways and the mechanism of regulation of signaling pathways by Leigongteng-related compound has not been elucidated yet,and it is urgent to explore the mechanism of efficacy of Leigongteng-related preparations on rheumatoid arthritis among multiple signaling pathways,and to further explore the potential value of Leiggongteng with the aim of providing certain references for the research on Leigongteng in treatment of RA.
