摘　　要：目的:观察补阳还五汤治疗甲状腺术后甲状旁腺激素≤5 ng·L^(-1)重度甲状旁腺功能减退症(hypoparathyroidism,HP)的临床疗效及安全性。方法:采用单盲、随机对照、前瞻性研究方法,125例甲状腺术后合并重度HP患者按照随机数字表法分为试验组(术后应用补阳还五汤联合补钙剂)66例,对照组(术后仅补钙剂)59例。治疗至甲状旁腺激素(parathyroid hormone,PTH)恢复至正常水平,最长随访周期不超过6个月。比较两组患者随访期总补钙量、PTH恢复至正常水平时间、PTH恢复率、有效率、不良反应发生情况。结果:治疗后,对照组所需总补钙量为(42.24±42.58)g,高于试验组的(17.71±12.33)g,差异有统计学意义(P<0.01);试验组PTH恢复至正常水平时间为(19.18±11.20)d,明显低于对照组的(39.93±39.43)d,差异有统计学意义(P<0.01);两组治疗有效率、6个月内PTH恢复率、随访期内两组不良反应发生情况比较,差异均无统计学意义(P>0.05)。结论:补阳还五汤治疗甲状腺术后重度HP疗效显著,可缩短PTH恢复时间,减少总补钙量,提高PTH恢复率,促进甲状旁腺功能恢复。Objective:To investigate the efficacy and safety of Buyang Huanwu Decoction in Treatment of severe hypoparathyroidism(HP)after thyroid surgery.Methods:A total of 125 patients with severe HP after thyroid surgery were divided into the experimental group(66 cases of postoperative application of Buyang Huanwu Decoction combined with calcium supplementation)and the control group(postoperative calcium supplementation only)(59 cases)according to the random number table method.Treatment until parathyroid hormone(PTH)returns to normal levels,and the maximum follow-up period is not more than 6 months.The total calcium supplementation,PTH recovery time,PTH recovery rate,total effective rate and adverse reactions of the two groups were compared during the follow-up period.Results:After treatment,the total calcium supplementation required in the control group was(42.24±42.58)g,which was higher than that in the experimental group(17.71±12.33)g,and the difference was statistically significant(P<0.01).The time for PTH to return to normal level in the experimental group was(19.18±11.20)days.It was significantly lower than that of the control group(39.93±39.43)days,and the difference was statistically significant(P<0.01).There were no significant differences in the effective rate of treatment,the recovery rate of PTH within half a year,and the number of adverse reactions between the two groups during the follow-up period(P>0.05).Conclusion:Buyang Huanwu Decoction has significant efficacy in treatment of severe HP after thyroid surgery,which can shorten the recovery time of PTH,reduce the total amount of calcium supplementation,improve the recovery rate of PTH,and promote the recovery of parathyroid function.
