罗库溴铵与苯磺顺阿曲库铵在结直肠癌根治术患者中的应用效果比较

摘　　要：目的:比较罗库溴铵与苯磺顺阿曲库铵对结直肠癌根治术患者肌松效果及应激反应的影响。方法:选取2023年6月—2024年6月于天津市第三中心医院分院行结直肠癌根治术的64例患者为研究对象,以随机数字表法将其分为对照组及研究组,各32例。麻醉期间,对照组采用苯磺顺阿曲库铵,研究组采用罗库溴铵。对比两组肌松效果、应激反应、血流动力学及不良反应发生情况。结果:研究组的肌松起效时间短于对照组,药物追加次数及恢复指数均低于对照组,维持时间高于对照组,差异均有统计学意义(P<0.05)。麻醉诱导后、插入喉罩后3min时,研究组的肾上腺素水平、去甲肾上腺素水平、心率及平均动脉压均低于对照组,差异均有统计学意义(P<0.05)。研究组的不良反应发生率为6.25%,略低于对照组的12.50%,但两组间比较,差异无统计学意义(P>0.05)。结论:罗库溴铵在结直肠癌根治术中较苯磺顺阿曲库铵具有更好的肌松效果和更低的应激反应,且在血流动力学方面表现更为稳定,不良反应更小,具有较高的应用价值。Objective:To compare the effects of rocuronium bromide and cisatracurium besilate on muscle relaxation and stress response in patients undergoing radical resection of colorectal cancer.Methods:A total of 64 patients who underwent radical resection for colorectal cancer in Branch of Tianjin Third Central Hospital from June 2023 to June 2024 were selected as the study objects and were divided into a control group and a study group by random number table method,with 32 cases in each group.During anesthesia,cisatracurium besilate was used in the control group and rocuronium bromide was used in the study group.The muscle relaxation effect,stress response,hemodynamics and the occurrence of adverse reactions were compared between the two groups.Results:The onset time of muscle relaxation in the study group was shorter than that in the control group,the times of drug addition and recovery index were lower than those in the control group,and the maintenance time was higher than that in the control group,the differences were statistically significant(P<0.05).After anesthesia induction and 3 min after laryngeal mask insertion,the levels of epinephrine,norepinephrine,heart rate and mean arterial pressure in the study group were lower than those in the control group,the differences were statistically significant(P<0.05).The incidence of adverse reactions in the study group was 6.25%,which was slightly lower than 12.50%in the control group,but there was no statistical significance between the two groups(P>0.05).Conclusion:Rocuronium bromide has better muscle relaxation effect and lower stress response than cisatracurium besilate in the radical operation of colorectal cancer,and has more stable hemodynamic performance and less adverse reactions,so it has higher application value.
