肺癌骨转移患者应用放射性核素示踪剂(^(99m)Tc-MDP)单光子发射计算机体层摄影/计算机体层成像骨显像联合正电子发射体层成像/计算机体层成像诊断的效能分析

摘  要:目的:分析肺癌骨转移患者应用^(99m)Tc-亚甲基二膦酸盐(MDP)单光子发射计算机体层摄影/计算机体层成像(SPECT/CT)联合正电子发射体层成像/计算机体层成像(PET/CT)诊断的效能。方法:对2022年4月~2024年3月本院收治的168例疑似肺癌骨转移患者的临床资料进行回顾性分析,两组患者均进行了^(99m)Tc-MDP SPECT/CT骨显像以及PET/CT显像,且间隔时间不超过2周。分析^(99m)Tc-MDP SPECT/CT骨显像、PET/CT显像以及两种显像联合的诊断价值。结果:^(99m)Tc-MDP SPECT/CT骨显像、PET/CT显像以及两种显像联合的敏感度分别为90.08%(109/121)、79.34%(96/121)、99.17%(120/121),差异有统计学意义(P<0.05);准确度分别为91.07%(153/168)、80.95%(136/168)、95.24%(160/168),差异有统计学意义(P<0.05);阴性预测率分别为78.57%(44/56)、61.54%(40/65)、97.56%(40/41),差异有统计学意义(P<0.05);三者特异度和阳性预测率差异无统计学意义(P>0.05)。结论:^(99m)Tc-MDP SPECT/CT骨显像联合PET/CT显像可以提高诊断的敏感度、准确率和阴性预测率,具有较高的诊断效能。Objective:To analyze the diagnostic efficacy of^(99m)Tc methylene diphosphonate(MDP)single photon emission computed tomography/computed tomography(SPECT/CT)combined with positron emission tomography/computed tomography(PET/CT)in patients with lung cancer bone metastases.Methods:A retrospective analysis was conducted on the clinical data of 168 patients with suspected lung cancer bone metastases admitted to our hospital from April 2022 to March 2024.Both groups of patients underwent^(99m)Tc MDP SPECT/CT bone imaging and PET/CT imaging,and the interval was not longer than 2 weeks.The diagnostic value of^(99m)Tc MDP SPECT/CT bone imaging,PET/CT imaging,and their combination was analyzed.Results:The sensitivity of^(99m)Tc MDP SPECT/CT bone imaging,PET/CT imaging,and the combination of the two imaging methods were 90.08%(109/121),79.34%(96/121),and 99.17%(120/121),with statistically significant differences(P<0.05).The accuracy rates were 91.07%(153/168),80.95%(136/168),and 95.24%(160/168),with statistically significant differences(P<0.05).The negative predictive rates were 78.57%(44/56),61.54%(40/65),and 97.56%(40/41),with statistically significant differences(P<0.05).There was no statistically significant difference in specificity and positive predictive rate among the three(P>0.05).Conclusion:The combination of^(99m)Tc MDP SPECT/CT bone imaging and PET/CT imaging can improve the sensitivity,accuracy,and negative predictive rate of diagnosis,and demonstrates high diagnostic efficacy.
