胎儿颈后透明层厚度联合四维彩超在早期产前胎儿畸形筛查中的准确率评估分析

摘  要:目的:探讨孕11~14周(+6d)胎儿结构畸形超声筛查的临床意义。方法:以2022年10月~2023年12月到本院进行超声筛查的孕妇66例为研究对象,以孕妇随机抽签分组方式平分为对照组和观察组,各33例,前者单独实施孕11~14周(+6d)胎儿超声测量胎儿颈后透明层厚度筛查,后者在此基础上增加四维彩超11~14周(+6d)胎儿结构畸形超声联合筛查,分析并对比两组胎儿结构畸形筛查情况,并总结性概述两组超声筛查应用意义与价值。结果:观察组胎儿结构畸形超声筛查总检出率明显较对照组更高(P<0.05);对比两组孕11~14周(+6d)胎儿结构畸形类别筛查有效率,观察组明显高于对照组(P<0.05);观察组孕胎儿结构畸形超声筛查灵敏度(78%)、特异度(54%)均较对照组灵敏度(22%)和特异度(46%)更高(P<0.05)。结论:胎儿颈后透明层厚度、四维彩超联合用于早期产前胎儿畸形筛查,效果佳,可有助于进一步提高胎儿结构畸形筛查效率及准确性,提高优生优育的成功率。Objective:To explore the clinical significance of ultrasound screening for fetal structural malformations at 11 to 14 weeks'age(+6d)of gestation.Methods:in October 2022 to December 2023 to our hospital for ultrasound screening of pregnant women 66 cases as the research object,to pregnant women randomly divided group and observation group,33 cases,the former alone pregnant 11~14 weeks(+6d)fetal ultrasound measuring after the neck transparent layer thickness screening,the latter on the basis of the increase four-dimensional colour to exceed 11~14 weeks(+6d)fetal structure malformation combined screening,analysis and comparison of two groups of fetal structure deformity screening,and summary overview of two groups of application significance and value of ultrasound screening.Results:The total detection rate of ultrasound screening of fetal structural malformations was significantly higher than that of the control group(P<0.05);the screening rate of 11 to 14 weeks(+6d)in the observation groups was significantly higher than the control group(P<0.05);the sensitivity of fetal structural abnormalities(78%)and specificity(54%)was higher than the control group(22%)and specificity(46%)(P<0.05).Conclusion:The thickness of the transparent layer behind the fetal neck and four-dimensional color ultrasound are combined for early prenatal fetal malformation screening,with good effect,which can further improve the efficiency and accuracy of fetal structural malformation screening,and improve the success rate of prenatal and postnatal care.
