摘　　要：目的:探讨超声引导下腹横筋膜平面阻滞用于子宫内膜癌手术患者的阻滞及镇痛效果。方法:收集2021年8月~2023年8月在本院行子宫内膜癌根治术的患者80例作为研究对象,参照随机数表法分为腹横筋膜平面阻滞(TFPB)组、腹横肌平面阻滞(TAPB)组各40例。对比两组患者的术后主观疼痛程度、术后自控镇痛情况、术后恢复情况差异。结果:术后6h、24h、48h,TFPB组患者的视觉模拟评分值低于TAPB组患者,差异均有统计学意义(P<0.05)。TFPB组患者的术后24h内舒芬太尼使用量少于TAPB组患者,术后首次自控镇痛按压时间晚于TAPB组患者,差异均有统计学意义(P<0.05)。TFPB组患者的术后排气时间、术后首次下床时间、术后总住院时间分别少于TAPB组患者,差异均有统计学意义(P<0.05)。结论:超声引导下TFPB用于子宫内膜癌手术患者,其镇痛效果优于TAPB,可更为有效地促进患者术后康复。Objective:To investigate the efficacy of ultrasound-guided lower abdominal transverse fascia plane block in patients undergoing surgery for endometrial cancer.Methods:A total of 80 patients who underwent radical surgery for endometrial carcinoma in our hospital from August 2021 to August 2023 were collected as research objects,and were divided into the TFPB group and the TAPB group with 40 cases in each group according to random number table method.The subjective pain degree,self-controlled postoperative analgesia and postoperative recovery were compared between the two groups.Results:At 6h,24h and 48h after surgery,the VAS scores of TFPB group were lower than those of TAPB group,with statistical significance(P<0.05).The amount of sufentanil used in TFPB group was less than that in TAPB group within 24h after surgery,and the time of first self-controlled analgesia compression after surgery was later than that in TAPB group,with statistical significance(P<0.05).The postoperative exhaust time,the first time to get out of bed and the total postoperative hospital stay of patients in TFPB group were lower than those in TAPB group,respectively,and the differences were statistically significant(P<0.05).Conclusion:Ultrasound-guided TFPB has better analgesic effect than TAPB in patients undergoing surgery for endometrial cancer,which can more effectively promote postoperative rehabilitation of patients.
